About the role

Location: Oakville – On-site role, based at the clinic’s front desk, supporting daily operations and patient interactions.

Key Responsibilities:

  • Coordinate new referrals from intake through to service initiation
  • Connect with clients to gather key information and provide clear, supportive explanations of the process
  • Document client needs related to physical, emotional, and cognitive challenges
  • Prepare and manage intake forms and digital documents, ensuring timely completion and follow-up
  • Maintain ongoing communication with clients and referral partners to provide status updates
  • Track the progress of each intake to ensure nothing falls through the cracks
  • Build strong relationships with legal, healthcare, and community partners to support seamless client care
  • Provide thoughtful, trauma-informed communication to individuals who may be overwhelmed or in distress
  • Support clients in understanding next steps and available services
  • Contribute to a high-quality, client-centered experience from first contact to service launch

Salary Range: $50,000

There are a multitude of different types of intervention services our team provides including: home/vehicle accessibility, equipment prescription, community safety/reintegration, wheelchair positioning/ seating prescriptions, activities of daily living (ADL) re-training/ functional restoration programming, cognitive remediation/compensatory therapies, cognitive behavioural therapy,  quality of life and return to modified sport programs. Our team is additionally skilled in vocational services, including return to work programs, work-site, vocational exploration and ergonomic assessments. ​
