As a Nurse Practitioner of the Palliative Care Program, you will provide diagnosis, treatment, pain and symptom management, counseling and coaching to clients – allowing for compassionate end of life care and the choice of death at home. As a recognized relationship builder, you will work in the aim of building capacity and competence in families, our diverse service partners, and physicians as part of an inter-professional model of palliative care. Sound knowledge of Ontario’s health care environment, with a deep understanding of client needs related to psychosocial oncology and palliative care. A CNA certification and Canadian Hospice Palliative Care Nursing Certification (CHPCN(C)) is considered an asset.

Essential Qualifications:

  • Master’s Degree in Nursing with registration in good standing, with the College of Nurses of Ontario
  • A current RN Extended-Class registration with the College of Nurses of Ontario
  • (Primary Care or Adult)
  • 3 to 5 years’ experience, preferably in a community setting and in Palliative Care Nursing
  • Adult teaching experience and/or adult education courses are an asset
  • Strong team leadership, relationship building, critical thinking and management skills
  • Demonstrated advance knowledge/experience in consultation and ethical decision making
  • Commitment to providing the highest level of client and family centred care
  • Superior oral and written communication skills
  • Demonstrated organization, time management, and problem solving skills
  • A passion for making a difference in the lives of palliative/oncology and their families
  • English/French bilingualism would be an asset for some positions
