Leading by example, the Manager of Process Improvement will work closely with Management and administrative teams (at times with direct reports) to ensure excellence, effectiveness and efficiency of service delivery is optimized through data driven continuous quality improvement.

As a Process Improvement Manager, You Will

  • Establish Best Practice guidelines and process for Care Coordination and all other functions within the region
  • Act as the project lead for process improvement, change management and local projects
  • Establish Process Improvement and Quality assurance plans and practice by collaborating with clinical leaders and leaders across different levels of organization
  • Develop project action plans and execute on timelines and deliverables
  • Assess and reassess project deliverables to ensure project plans align with project goals
  • Maintain a comprehensive strategy and plan regarding change management
  • Identify opportunities to innovate, improving service delivery, process and/or systems
  • Ensure all processes and procedures associated with accreditation are followed and consistently documented
  • Work closely with clinical leaders to ensure alignment between clinical Best Practice and process Best Practice
  • Manage data associated with identified Key Performance Indicators (KPIs) and provide data analysis to identify
    opportunities for improvement
  • Identify opportunities to improve service and communicate to those who can use the data to make appropriate changes
  • Analyze and interpret data to provide feedback to the Regional Director, Care Managers or other relevant internal stakeholders

The ideal candidate for the Process Improvement Manager role must have

  • A Post-Secondary education in a related field (Business Administration, Project Management)
  • At least 3 years of progressive leadership experience
  • Demonstrated experience and success in project planning and implementation
  • Experience in data analysis to drive quality improvements
  • Strong computer literacy and skills with various applications
  • Be able to work independently and also as an effective member of a team
  • Have excellent interpersonal and communication skills (written and verbal)
  • Experience or a strong understanding of the Home Health sector is strongly considered an asset.
