Join our compassionate healthcare team as a Care Manager in our Private Care department, where we provide exceptional care and every day brings the opportunity to help clients live safely, comfortably, and independently in their own homes.

As part of our healthcare team, you'll be the heart of our in-home care services - coordinating personal support for clients in the community and ensuring they receive the high-quality, compassionate care they deserve. You'll build relationships, lead with empathy, and make a real impact every single day!

Acclaim Health announces the availability of a Temporary Full-Time Care Manager, Private Care position with the Private Care and Customer Support department working in the Oakville Office for a period of six (6) months. The incumbent will work Monday to Friday from 8:00am - 5:00pm.

Pay Range: $32.70 - $34.68

PRIMARY FUNCTIONS:

  • In collaboration with Human Resources, manages the recruitment of new private care staff

  • Effectively manages the performance of direct reports through direct monitoring and regular performance reviews

  • Provides input into the development of education plans

  • Ensures that direct reports understand and are aligned to their key accountabilities and perform to the expected level

  • Leads and facilitates team meetings

  • Oversees daily and monthly scheduling of private care staff

  • Provides internal customer service excellence through leading by example utilizing coaching and mentoring to support employees in providing excellence in patient care

  • Effectively manages time off work, including vacation requests, sick time and LOA requests

  • Provides evening and weekend on-call service for Health Services on a rotational basis

  • Participates and collaborates with a multidisciplinary care team including staff, patients and caregivers in supporting the patient’s needs according to the patient’s care plan

  • Works in collaboration with the Service Coordination Team to ensure patient-focused scheduling

  • Responds to client enquiries, concerns, changing client needs and ensures ongoing client satisfaction and safety

  • Other duties as assigned

QUALIFICATIONS & EDUCATION:

  • Baccalaureate Degree, Nursing/Practical Nursing diploma or equivalent

  • Valid Ontario driver’s license, use of vehicle and insurance per Acclaim Health policy

EXPERIENCE & SKILLS:

  • Minimum 3-5 years as a Registered Nurse or Registered Practical Nurse

  • Supervisory or leadership experience an asset

  • Demonstrated ability to coach, mentor and lead staff

  • Effective verbal and written communication skills

  • Demonstrated assessment, problem-solving and negotiating skills

  • Strong planning, coordinating and organizing skills

  • Demonstrated ability to work independently and collaboratively as a team member

  • Demonstrated knowledge and skills in quality/risk management

  • Strong customer service skills

  • Proven ability to multi-task and meet deadlines

  • Proficient in Microsoft Office, HPG and AlayaCare Knowledge of community resources
