The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

Qualifications:
- Member in good standing with a current Certificate of Registration with a Regulated Health Professions College in Ontario required
- Nursing or social work/social service background preferred, other allied health professionals will be considered
- Indigenous lived experience, required
- Preference will be given to someone with experience working with a First Nation(s) community
Principal Duties:
Through early engagement with the patient, family and care team, this position assists with leading patient transitions from admission to discharge, coordinating care and services between various non-Indigenous health providers and Indigenous service providers or community-based health services. This role provides care coordination/case management, system navigation, education, access to culturally appropriate health care and quality improvement processes.
- Assist Indigenous patients, caregivers and their families to better navigate the health care system by assisting with the coordination and integration of the care between health care providers and Indigenous service providers, community-based health services
- In consultation with the client and the health care team, individualize the care path to reflect interventions which meet identified health care needs while promoting cultural appropriateness, safety, privacy and confidentiality for clients
- Guide patients and families with an understanding of the care plan, hospital services, and community-based services
- Work collaboratively with First Nation Home and Community Care programs, Ontario Health at Home hospital coordinators, social work and therapy services to support successful discharge
- Develop and maintain respectful working relationships with Indigenous services/organizations, members of the health care team and various hospital services
- Facilitate and strengthen linkages to palliative care services, as well as, mental health and addictions, chronic disease management, primary care and acute care services
- Through transitions of care, ensure appropriate services and resources are available and implemented for successful patient outcomes
- Maintain appropriate documentation of patient care planning in the electronic medical records system
- Support and maintain data collection for the purpose of reporting and improving quality of care and services
- Support clients, caregivers and families in communicating with physicians, nurses and members of the health care team,Traditional Healers, Elders, and Knowledge Keepers
