Clinical Pharmacists - VH
Reporting to the Manager, Clinical Pharmacists, the Clinical Pharmacy Specialist functions as an expert Practitioner, Consultant, Educator, and Mentor to Pharmacists at Victoria Hospital. The Clinical Pharmacy Specialist develops, plans, organizes, and evaluates clinical practice, education, and research activities both within the Pharmacy Department and externally, including teaching to Residents, undergraduate, graduate students, and Pharmacists. This role ensures the highest standards of professional practice, quality, and safety are maintained within the Pharmacy Department. The Clinical Pharmacy Specialist also covers a clinical service and participates in distribution activities verifying patient orders.
Assignment: Adult Psychiatry
Regular Full-Time
Hours of work: 37.5 hours per week (guarantee of hours)
Rate of pay: $58.15 per hour - $72.68 per hour
Qualifications
- Baccalaureate Degree or Doctorate Degree (PharmD) in Pharmacy
- Registered as an A Pharmacist with the Ontario College of Pharmacists (OCP)
- Professional Liability Insurance as prescribed by the OCP
- Hospital Pharmacy Residency Certificate (Accredited Canadian Pharmacy Residency: ACPR) preferred
- At least three (3) years recent experience in a hospital setting
- At least two (2) years recent, relevant experience working with psychiatry patients
- Five (5) years recent, relevant experience preferred
- Proficiency in OCP Standards of Practice for Pharmacists
- Advanced literature appraisal skills
- Advanced ability to coach, mentor, teach, demonstrate techniques, and facilitate learning within a multidisciplinary setting
- Experience with Cerner PharmNet and Powerchart System
- Demonstrated excellent communication (verbal and written) skills
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC's vision, purpose and values
- Demonstrated ability to attend work on a regular basis
