Job Description
POSITION SUMMARY
The Project Support Specialist provides ongoing support for the implementation, monitoring and evaluation of improvement initiatives and projects primarily in Long-Term Care and throughout the division to assist Operational Departments and Management. The Project Support Specialist is responsible for project administration, document management, scheduling meetings, tracking progress, reporting, communication and delivering on key strategic outputs and milestones. Ensuring projects run smoothly and efficiently, the Project Support Specialist will assist operations to meet legislative, professional practice & customer service requirements, and contributes to an effective, responsive, and accountable Health Quality Simcoe department.
DUTIES AND RESPONSIBILITIES (not listed in order of priority)

Using best practice project management methods, assist project management activities by tracking and monitoring project milestones in project charters, project plans, action plans and annual priorities supporting the schedule of deliverables and project timelines.

Assist with documentation management activities by creating, maintaining and organizing project documents, records and reports.

Support projects, action plans and annual priorities through the gathering and documenting requirements for deliverables, data collection, and needed reporting to measure success of quality improvement activities, and outcomes.
-
Prepare regular project status reports reporting on milestones and timelines; schedule and conduct project status meetings with project teams; identify and report on project issues; and provide regular briefings and updates during project life cycle.

Facilitate communication between team members and stakeholders, including scheduling meetings and distributing information.
-
Monitor and track action planning in response to findings from service evaluations, inspections and audits.
-
Support department steering committees while acting as the liaison and project representative between project leads and the department key stakeholders, as well as with departments from other divisions.

Support Quality Improvement initiatives through utilizing Lean process improvement tools such as: Root cause analysis, Value Stream Mapping; Plan-do-study-act cycles, time analysis and resource allocation.

Assist in gathering information for assessments against global standards, e.g. Accreditation Canada and PS Service Review.

Support the development, coordination and tracking of action plans and annual requirements for global standards e.g. Accreditation Canada and PS Service Review
-
Assist with the coordination, tracking and communication of Health and Emergency Services surveys and evaluations.
-
Work with Project Coordinators to research and identify potential grant funding opportunities for Health and Emergency Services.
-
In collaboration with Operations, provide support for the ongoing revisions to processes and procedures to maintain a high standard of care to benefit the clients, residents, staff, families, and volunteers.

Provide input to the Manager, Professional Practice & Client Experience for the Health Quality Simcoe department strategic and business planning, as appropriate.
-
Develop and maintain cooperative and collaborative relationships with internal and external business partners.
-
Participate in and demonstrate an understanding of patient safety principles and practices in all day-to-day activities. Follow all safe work practices and procedures and immediately communicate any activity or action which may constitute a risk to patient safety.

Participate in committees as required.

Perform other duties as assigned, including redeployment in emergency situations.
-
Work in compliance with the Occupational Health & Safety Act, and the Fixing Long Term Care Act, the Ambulance Act and their regulations in performing duties in a safe manner and follows all County of Simcoe Health & Safety policies, procedures and legislation.

Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act, and the Personal Health Information Protection Act.

EDUCATION, TECHNICAL SKILLS AND CERTIFICATION

A diploma or degree in Health Sciences, Quality Management, Business Administration. This job may be considered for educational equivalency in accordance with County Policy.

Certificate in project management or ability to obtain within six months of hire
-
Completed Lean Certification (Yellow Belt or higher) the Canadian Healthcare Association (CHA) program on continuous quality improvement, the Certified Professional in Healthcare Quality (CPHQ) or an equivalent program preferred.
-
Strong communication skills both written and verbal to effectively communicate information to the interdisciplinary team.
-
Presentation skills are an asset.
-
A working knowledge of computers, their application and related software essential to generate necessary reports.
-
Organizational skills to prioritize work and ensure deadlines are met.
-
Valid G class drivers license and access to reliable vehicle.
-
Criminal Records Check with Vulnerable Sector Screening that is less than six (6) months old at time of hire.
-
As a condition of employment, candidates are required to submit documented results of TB testing, as per Public Health requirements.
EXPERIENCE
-
Minimum one (1) year experience in healthcare, preferably in Paramedic Services or Long-Term Care, including one (1) year experience in project management or process improvement or an equivalent combination of education and experience.

Demonstrated knowledge and understanding of the demographics and populations at risk, within the County of Simcoe.
-
Experience supporting project teams.
