Role Overview:
In supporting individuals with disabilities and complex needs within the designated Support Homes and IHACSS programs, our primary objective is to facilitate their success in achieving goals related to home living, life skills, daily living, and community inclusion. This role is centered on practicing and promoting high standards of care and professional responsibilities.
As a key contributor, you will serve as a positive role model and advocate for individuals receiving services from, I Have A Chance Support Services Ltd. Your responsibilities include ensuring their safety within both their home and the broader community, and addressing any concerns that arise. You will support individuals in understanding their personal rights, the rights of others, and the impact of their behavior on those around them. Additionally, you will guide them in recognizing and reporting situations of suspected abuse.
Fostering the development of relationships and supporting community inclusion are also crucial aspects of this role. You will assist individuals in becoming active, valued, and contributing members of the community, while also aiding them in understanding and solving problems and developing essential skills. Maintaining a thorough understanding of each individual's needs and establishing professional relationships based on respect and dignity are fundamental to providing effective support.
Creating a homelike environment that emphasizes daily living skills is a priority, along with supporting individuals during times of crisis and challenging behaviors. Your role includes providing health and daily living supports, addressing complex and challenging behaviors that may impact mental health and well-being, and implementing enhanced lifestyle and daily living skills interventions as needed.
Additionally, you will support and guide employees to succeed in their roles by role-modeling IHACSS expectations and ensuring adherence to organizational policies and procedures. This position demands a proactive and respectful approach to care, underpinned by a commitment to the overall development and well-being of the individuals serve
Key Responsibilities:
- Maintain a working knowledge of everyone in care and a professional relationship based on respect and dignity
- Provide support services to implement all activities and develop in-house programs in collaboration with the Client Care Manager or designate
- Provide support services to individuals regarding maintaining a household, personal health, developing relationships, managing finances, active community inclusion and designated programs.
- This may include Meal planning, preparation, household cleaning, transportation and other duties as required Encouragement and facilitate individuals to achieve success in their Individualized Care Plans (ICP)
- To support individuals to be safe within their home and in the community and to identify and deal with any concerns and crisis situations
- To support individuals to understand their personal rights, the rights of others, how their behavior impacts others, and how to identify and report situations of suspected abuse
- To support an individual’s freedom of belief, faith, religion, and culture and assist individuals to practice these as desired
- To support individuals in the development of healthy and positive relationships
- To support community inclusion and to assist individuals to become active, valued, and contributing members of the community
- To assist individuals in understanding, problem solving, and the development of skills
- Administer prescribed medications as directed
- Facilitate and develop daily program schedules as required
- Provide transportation for the individual in care using IHACSS Fleet Vehicles
- Maintain IHACSS Fleet Vehicles and abide by all IHACSS policy and procedures regarding use
- Promote individual in care safety and wellbeing practices
- Role model appropriate and healthy behaviors and practices while on shift
- Ensure all medical appointments and health concerns are followed through and documented accordingly
- Ensure Support Homes are kept clean, orderly, and all hazards are eliminated or controlled
- Ensure regular fire drill, smoke detector, and inspections of premises/equipment are completed
- Provide crisis intervention and physical support as trained and certified in. Role-model appropriate skills and support employees to develop skills
- Support individuals in care through behavioral de-escalation and pre-behavioral engagement
- Engage in post-incident debrief with individuals in care and employees
- Support safety procedures and ensure the health and wellbeing of all individuals in care and employees during time of crisis
- Provide after-hours On-Call Support as outlined in IHACSS policy
- Coordinate and attend all mandatory Support Home Team Meetings (Monthly) and create meeting minutes
- Maintain employee personal file with required (and accurate) credentials
- Attend all mandatory training and uphold certifications
- Participate in accreditation and vendor audits as required
Qualifications
- Degree or diploma in Health Services, Human Services, Disability Services, Correctional Services or a relevant field (Competency-based hiring may be considered in lieu of a degree or diploma).
- 3 years of related experience working with the population served.
- 2 years of related leadership experience within support services.
- Possession of a valid driver’s license and a reliable vehicle.
- Clean driver abstracts and document proof of insurance.
- Employees must be able to drive agency fleet vehicles with proficiency.
- Vulnerable sector check and child intervention record check.
- Proficiency in English reading and writing skills.
- Demonstrated aptitude in working with diverse populations.
- Proficiency in collaborative teamwork.
- Working knowledge & implementation of regulatory traffic laws.
- Code of conduct of caregiver ethics and morals.
- Flexibility to accommodate various shifts, including weekends and overnights.
