Are you an experienced Social Worker (SW) who has a passion for complex & palliative care? Do you have an interest in ensuring healthcare professionals are educated and supported in best practice? If you’re looking to make the difference in the lives of Albertans and impact the care they receive, the position of Social Worker II on the North Zone Specialty Programs - Palliative & Complex Care Support Team could be for you. As a SW, you will play a key role in the care of clients served in Continuing Care within the North Zone. You will be a member of a mobile/virtual interdisciplinary team providing consultation services for palliative and complex care clients. By using a client and family-centered care approach, you will work with local clinicians and other teams to provide specialized assessment, care planning and follow-up for clients in a consultative and limited direct service model. In this role, you will provide coaching and education to other care team members to build capacity in the care of client with palliative and end of life care needs. There is a strong focus on multidisciplinary and collaborative processes. You will participate in assessments, case reviews & consultation with other team members. You will liaise with representatives in Continuing Care, Home and Community Care, Acute Care, community agencies & others for assessment and care planning. You will use information gathered from direct care staff to identify system and client specific barriers and provide leadership in system navigation. This position supports clients in the North Zone, located out of Grande Prairie. Location may be negotiable. This position includes virtual work and in-person client consultations. Travel (of up to 2 hours) is required.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum 5 years recent Social Work clinical experience. Minimum 1 year experience, within last 5 years, working with palliative care clients. Minimum 1 year experience, within last 5 years, working in Home & Community Care and/or a hospice setting. Minimum 1 year experience, within last 5 years, Continuing Care, Brain Injury, Mental Health, or other relevant clinical area. LEAP and/or palliative care training. Competent with MS Office Suite, TEAMS, ZOOM, Tele-health. Valid driver's license and access to a motor vehicle in good repair for work-related travel. Able to work independently in different environments (weather conditions). Must be physically capable of carrying approx. 25-30lbs. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

Preferred Qualifications:

Knowledge of Continuing Care service, including delivery options, assessment/placement criteria, and community supports and agencies. Connect Care trained. Additional preparation in Psychiatry, Mental Health and/or Palliative/end of life care. PIECES and/or Supportive Pathways trained. Completion of McMaster Case Management training - Level II. Previous experience with providing adult education.
