UNION: ONTARIO NURSES' ASSOCIATION WAGE RANGE: $69.960 TO $79.910 PER HOUR
POSITION OBJECTIVE: The Nurse Practitioner will work in collaboration with the Niagara Health family medicine program. The Nurse Practitioner is responsible for ongoing assessment, planning, management and treatment of patients within the complex care unit in collaboration with the Fort Erie Physicians. The Nurse Practitioner will report to the Program Director. The Nurse Practitioner will provide leadership and promote excellence in patient-centred care through expert practice, education, consultation and research. The successful candidate will provide care with an emphasis on wellness, prevention and disease management through direct patient care, health promotion and education, outreach, advocacy and community development as required. The successful candidate's home site will be the Fort Erie Site, rotating to other Niagara Health sites as required.
QUALIFICATIONS: o Current registration in good standing as a Registered Nurse Extended Class Primary Health Care or Adult Certification with the College of Nurses of Ontario (CNO) o Completion of Nurse Practitioner Program and/or Master in Nursing or Master of Science in Nursing o Must have successfully completed CNO approved controlled substances education o Must have professional Liability Protection (RNAO or equivalent) o Must have experience working as a Nurse Practitioner in Acute Care, Complex Care, or Long Term Care settings o A minimum of 5 years clinical experience as a Registered Nurse in Acute Care, Complex Care, or Long Term Care settings o Clinical expertise in gerontology o Demonstrated clinical leadership skills to organize and coordinate the care of patients in collaboration with patients, families, and the health care team o Ability to promote professional education and advanced knowledge through research and evaluation/quality improvement activities related to care of patients throughout the continuum. o Evidence of effective organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills. o Demonstrated skill in program development using a collaborative, stakeholder engaged process o Ability to be self-directed in clinical practice as required in the areas of assessment, diagnosis and therapeutics o Applies Niagara Health values and professional practice standards to patient/client care and service o Demonstrated commitment to patient/client centred care o Strong analytical and conceptual skills required o Must be willing to work flexible hours to reflect patient population needs o Consistent and regular attendance required.
