About the role:
The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)
is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values
and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures
continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,
understanding and commitment to this care philosophy.
The Patient Flow Coordinator, Hospital to Home (H2H), under the direction of the Manager, Hospital to Home, and
Director, Patient Flow & Partnerships with matrix reporting to the Director, Nursing & Health Professions, is
responsible for planning and coordinating the day to day activities related to patient flow and interprofessional
teamwork within the H2H program. The Patient Flow Coordinator is responsible for the overall processing of new
patient referrals as well as coordinating and facilitating adequate home and community care services of patients at
home. The Patient Flow Coordinator works within the program leadership team that includes the H2H manager,
operations coordinator, nurse practitioner, and home and community care team members.
EMPLOYMENT REQUIREMENTS:
Qualifications:
* Current unrestricted registration as a Registered Nurse with the College of Nurses of Ontario.
* Prior experience as a Patient Flow Coordinator and/or home and community care coordinator considered an asset.
* Demonstrated leadership experience and collaborative leadership style.
* Discharge planning experience, specifically in the coordination and delivery of services and knowledge of patient
flow concepts.
* Effective communication skills, including the ability to provide constructive feedback and resolve conflict.
* Demonstrated ability to build relationships and work within a team.
* Knowledge of team?building principles and interprofessional collaboration.
* Knowledge of professional practice standards and guiding principles to foster professional practice and
accountability.
* Knowledge of Community Agencies, Long Term Care and other health care resources within the community and
region.
* Adherence to PFCC philosophy, knowledge of CNO standards of practice.
* Ability to work under pressure and manage competing work demands.
* Proficiency in technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word, Teams,
PowerPoint, and Excel.
* Regular and sustained attendance as per hospital standard.
* Knowledge and adherence to the hospital's Code of Conduct.
Salary Range: $45.150 to 62.980
The Health Sciences Centre strives to ensure the safety and security of the patients, visitors, employees and assets

Vulnerable Sector Check (VSC) where indicated, to ensure the absence of relevant criminal convictions; and proof of
full vaccinations approved by Health Canada to Occupational Health & Safety. Candidates will have demonstrated
adherence to the Hospital's Code of Conduct. Regular attendance at work is imperative, therefore, all applicants will
have to demonstrate a good attendance record to be considered for this position.
