PRIMARY FUNCTIONS:

• Supports patients/clients by providing in-home companionship and caregiver respite, supervision in the personal care, housekeeping, meal preparation, and/or pet care as required
• Driving patient/clients to and from appointments, social events and/or for errands
• Participates and collaborates with a multidisciplinary care team (including staff, patient and caregivers) in supporting the patient’s needs according to the patient’s care plan.
• Acts as an effective and professional liaison between the patient/client/caregiver and Acclaim Health office (including recommending referral to other Acclaim Health services as appropriate).

POSITION REQUIREMENTS:

• Valid Ontario Driver’s license and access to a vehicle
• Current First Aid Certificate
• Safe Food Handling Certificate is an asset
• Successful completion of Acclaim Health’s Companion Training Program (includes procedures for all client services, Gentle Persuasive Approach (dementia related education)
• Criminal Record check with Vulnerable Sector Screening
• Experience working with the elderly, ill or with children
• Good written and verbal communication skills
• Effective observation and listening skills
• Demonstrated ability to work independently and collaboratively as a team member

SALARY & COMPENSATION:

• CA$17.56 - $18.11 per hour
