About Us
The Prince Edward Family Health Team (PEFHT), funded by Ontario Health, offers comprehensive patient-centred care for Prince Edward County residents and surrounding area residents. Services and programs include disease diagnosis, treatment and management, prevention and cure, rehabilitation, palliative care, and community health promotion.
The Role
As a Clinical Pharmacist, you will play a vital role in delivering comprehensive primary care. Your expertise will support health promotion, chronic disease management, medication optimization, and patient education, ensuring exceptional, integrated care as part of a multidisciplinary team.

Provide education on pharmacological therapies, promoting adherence and understanding of treatment plans.

Offer evidence-based consultative advice to patients and team members on medication management.

Support medication reconciliation in collaboration with local pharmacies and healthcare agencies.

Manage complex medication regimens for acute and chronic conditions.

Identify and resolve actual and potential drug therapy problems.
-
Collaborate with the interprofessional primary care team to develop, implement, and evaluate care plans.

Act as a drug-related information resource, providing updates on guidelines, safety alerts, and pharmacotherapeutic advancements.

Contribute to the development, planning, implementation, and evaluation of clinical programs that enhance patient outcomes.

Provide support to patients of other PEFHT programs such as Memory Clinic, Diabetes Education, Cardiac Rehab, and Smoking Cessation, and Community Clinic to ensure optimal drug therapies are achieved.

Design, develop, and deliver group patient presentations on specific therapeutic topics based on patient needs.

Drive quality improvement initiatives to optimize care processes and outcomes.

Maintain accurate and comprehensive documentation in the EMR system (Accuro).
Required Qualifications

Minimum Honours Baccalaureate degree in Pharmacy.

Active in Part A of the register with the Ontario College of Pharmacists.
-
Minimum 2 years of experience in primary care or community-based healthcare preferred.

Proven ability to work within a multidisciplinary team and independently.

Strong communication, problem-solving, and conflict resolution skills.
-
Proficiency in EMR systems and patient outcome data analysis.
-
Commitment to providing equitable and culturally sensitive care.
-
Proof of liability insurance.
-
Valid driver’s license, access to a vehicle, and appropriate insurance for travel.

Satisfactory Vulnerable Sector Screening.
-
Interest and willingness to be certified in CBT-I to counsel patients on insomnia and sleep-related pharmacotherapy would be an asset.
