About the role

Case Manager – Mental Health Court Support Program (Full Time Permanent) The Role and Opportunity The Canadian Mental Health Association is looking for a Case Manager for the Mental Health Court Support Program . The Court Support Program works with individuals living with mental illness who have criminal charges pending with in the Court. These individuals may be diverted from the court system and connected to mental health and support services in the community. Court Support Services also provide consultation for individuals who do not qualify for diversion, linking them to services to facilitate bail or to assist with sentencing.

Salary: $54,943 – 60,203

Hours of Work: 35 per week

Responsibilities Reporting to the Program Manager of Court Support, the Case Manager will provide community-based support and court-based supports to individuals with serious mental health and addictions needs who are involved with the criminal justice system.

The aim of the Case Manager will be to working closely with court support staff and other service providers in the community as well as linking individuals to needed supports and services and advocating for the clients on their behalf . In addition, information and consultation is provided to lawyers, the judiciary, probation and bail supervisors, and families. T he role will also be required to support the client by providing orientation to the program and agency and to develop release and diversion plans, including conducting regular evaluations and follow-ups as needed.

The role will involve case management duties where the Case Manager will build and maintain a trusting respectful and collaborative working relationship with client, including client-centered engagement, service provision, interventions, skills development and goal planning.

Experience & Education Required CMHA Toronto’s ideal candidate will have a Diploma in Social Work/Social Science, or other health- related disciplines. The candidate will bring a minimum of three years of direct service delivery in criminal justice and/or, mental health field . Knowledge and experience of concurrent disorders, homelessness, recovery principles and the criminal justice system as it relates to mental health

The candidate will bring strong assessment skills with a solution- focused approach to work and the ability to manage crisis, as well as ability to work with diverse communities and demonstrate a non- homophobic and anti- racism approach .

The candidate will also be required to have a valid Ontario Driver’s License, access to a vehicle and insurance coverage.
