Salary: $87,536 to $109,420 per annum
Description:
Hillsdale Terraces is currently seeking a Registered Dietitian to join its team.
The incumbent will:
- Manage the nutritional needs of residents through nutritional risk screening, assessments (admission and routine), referrals, nutrition care planning, monitoring established interventions and evaluation of outcomes
- Oversee the nutrition/hydration component of the facility's enteral feeding program
- Participate in care conferences with residents and their healthcare advocates
- Address and follow up on any referrals and/or reported nutritional care concerns in a timely manner
- Develop, implement and maintain policy and procedures specific to the nutrition/hydration program
- Maintain accurate, organized and secure clinical records for all residents within the home
- Develop and approve regular, textured and therapeutic menus to ensure nutritional adequacy, food safety and palatability and compliance with established menu planning standards
- Plan and conduct health teaching to residents and families, and educate staff and volunteers on nutrition related topics
- Assist the Food Services Manager in the quality assurance and management of the department and participate in quality improvement initiatives within the Division/home
- Mentor students during clinical/food services placements as deemed necessary
- Actively participate on various committees (i.e.: Food Service, Skin/Wound Care, Pain and Palliative Care and Professional Advisory Committee, Interprofessional Quality and Resident Safety Committee and Peer Group meetings for the Division)
- Work in accordance with the provisions of applicable health and safety legislation and all corporate/departmental policies and procedures related to occupational health
The successful applicant will possess:
- A Bachelor of Science or Applied Science with a Major in Food and Human Nutrition
- Current registration in good standing with the College of Dietitians of Ontario
- Several years experience in a long-term care setting with sound knowledge of geriatric nutrition and dysphagia management
- Demonstrate a willingness to adapt programs/services to meet the diverse needs of the resident population
- Excellent interpersonal and communication skills, with the ability to effectively resolve conflicts
- Demonstrated organization, project management and time management skills
- Exceptional clinical assessment skills
- Demonstrated ability to work independently within an interdisciplinary healthcare environment
- Knowledge of Fixing Long Term Care Act and Regulations, Personal Health Information Protection Act (PHIPA), Health Care Consent Act and Accreditation Canada Standards
- Proficiency with various software, including MS Office applications
- Knowledge of Point Click Care, Synergy MealSuite applications and Long Term Care Facilities (LTC-F) InterRAI is preferred
Conditions of Employment
The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within two (2) months from date of hire. Additionally, tuberculosis screening will be required in accordance with the FLTC Act (O.Reg. 246/22). Proof of education, qualifications and any other job bona fide requirements will also be collected.
