ABOUT THE ROLE
Reporting to the Vice President, the Regional Referral System Manager is responsible for the development of a coordinated regional referral model using a single access point to provide equitable access to chronic disease programs and services across sectors, settings and communities across Northwestern region. Key responsibilities include leveraging existing digital health infrastructure to standardize and expedite access to care; engaging key stakeholders; and identifying relevant infrastructure and business solutions necessary to advance coordinated referrals. The Manager will provide leadership and engage organizations involved in the delivery of chronic disease services throughout Northwestern Ontario to participate in standardization of referral process pathways and tools design.

WHAT YOU BRING
Education/Experience:
-
Degree in information systems/health sciences/health promotion or clinical-related field or an equivalent combination of education and experience
-
Leadership experience with the ability to manage complex projects involving multiple agencies, budgets and stakeholders
-
Leadership experience with the ability to manage people and system operations within SJCG
-
Experience developing, implementing and sustaining network/partnerships
-
Strategic planning and project management experience, including demonstrated business skills, as well as experience in program and system evaluation
-
Experience in implementation of information systems
-
Working knowledge of digital health principals and existing referral management software
-
Knowledge of health information system integration and redesign provincially
-
Knowledge and/or experience of chronic disease services and service delivery models (i.e. regional, provincial and national policy/funding issues)
-
Knowledge of the current and changing dynamics of the health care system, related support services, and the roles of health professionals in chronic disease management
-
Demonstrated knowledge of community resources and community needs
Skills/Abilities:

Ability to communicate effectively and concisely, both orally and in writing

Proven ability to establish and maintain co-operative and collaborative relations with private and public officials, co-workers, and other stakeholders

Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills

Strong leadership skills, including the ability to motivate and recognize others; proven ability to share skills and knowledge with others

Conditions of Employment:

This position shall be conditional upon a satisfactory Criminal Record & Judicial Matters Check (CRJMC) where indicated, to ensure the absence of relevant criminal convictions.
