POSITION SUMMARY:

Following Housing First principles, and as part of Mission Services’ Night Link team, the Early Intervention Worker will provide homelessness prevention, diversion and late stage homelessness support. The Early Intervention Worker will work as a part of the Night Link team and will engage regularly with service users to provide the necessary support to achieve program and participant goals. The Early Intervention Worker will work with individuals of all genders over the age of 18.

KEY RESPONSIBILITIES:

  • Conduct assessments by phone and in person to determine the needs and resources of each individual, in order to develop a short-term service plan to support each individual’s specific needs
  • Ensure the health and safety of all people accessing the Night Link Program, addressing immediate needs and providing direction to individuals accessing the space
  • Utilize de-escalation and empathetic listening skills when engaging with individuals
  • Complete intakes and track individuals accessing the space; input this information properly and accurately
  • Assist in the set up and take down of the drop-in space. Maintain program supplies and report to manager as needed
  • Respond to crisis situations involving service users by assessing the situation and implementing appropriate solutions which promote service user safety
  • Apply homelessness prevention and shelter diversion strategies where possible, to assist services users to sustain their current housing and/or be directed to services that meet their needs
  • Collect and assess information to determine service user’s eligibility for emergency shelter and other shelter and housing related services
    • Coordinate referrals to community agencies, City of Hamilton programs, and other levels of government to meet service user needs
  • Utilize the principles of case management when conducting assessments and referrals to address services user needs
  • Advocate on behalf of service users for services and benefits with internal and external resources and supports, income maintenance programs, and shelters and other service providers
  • Complete third part verification by contacting various sources, including community agencies, housing providers, health care providers, income maintenance systems, child welfare agencies, Public Trustees, and other service providers
  • Maintain thorough case documentation including documenting all intake and case plan information and entering statistics. Update and revise intake records with each service user contact to ensure accuracy
  • Maintain a high level of familiarity with legislation, community services, resources, programs, policies and procedures, and issues affecting or related to the service user population
  • Maintain a high level of confidentiality related to all service user matters and information
  • Participate in team meetings and case conferences to discuss program issues, review service user needs, discuss case plans, homeless prevention/shelter diversion plans, etc.
  • Work safely, and identify and report any health and safety concerns, in accordance with Mission Services’ Workplace Health and Safety Policies
  • Adhere to a strict confidentiality policy
  • Other duties as needed to ensure the continuation of services

QUALIFICATIONS:

  • Post-secondary diploma or degree in human or social services, or equivalent experience
  • One to two (1-2) years of experience in Early Intervention or Emergency Shelter Work, specifically experience working with adult males, females and non-binary individuals experiencing homelessness, mental health and addictions concerns
  • Experience in case management techniques, use of, and connection to, community resources including housing providers, income maintenance programs, shelters and other service providers
  • Ability to provide person-centered, non-judgemental supports to others
  • Ability to form helping relationships based on trust and empowerment
  • Valid Non-Violent Crisis Intervention and First Aid certification is an asset
  • Excellent interpersonal skills

HOURS OF WORK:

  • 37.5 hours per week, afternoon shifts
  • The successful candidate must be available to work weekends

REQUIREMENTS:

  • Police Vulnerable Sector Check
  • Two-Step Tuberculosis Skin Test
