Role: Pharmacy Student
To provide direct patient care in the management of drug therapy to achieve positive patient health outcomes, primarily for patients of a medical-type inpatient unit, and coverage to other area as needed. Direct patient care focuses on review of medication orders; selection and assessment of patient´s requiring pharmaceutical care; identifying, resolving and preventing drug therapy problems for a subset of patients; patient education; patient continuity of care; and the provision of drug information.
To facilitate and promote accurate, safe, appropriate and cost effective use and distribution of drugs throughout the hospital, including weekend, evening and on call rotations.

Position Summary
- Gather Best Possible Medication Histories from admitted patients and enter them into the electronic health record (EPIC)
- Coordinate with Pharmacists and Allied Health team for clarification of specific medications
- Participate in projects and safety initiatives.
- Other duties as assigned.
Qualifications

Enrollment in a University Pharmacy Program required.

Experience with EPIC and Best Possible Medication History gathering strongly preferred.

Computer skills required.

Demonstrated ability to work in a fast paced environment.

Team player, reliable, accurate and able to work efficiently to meet deadlines.

Self-motivated team player, reliable, cooperative, and patient-focused.

Excellent interpersonal and communication skills.
