Job description:

Job Summary

Reporting to the Administrator, this position is responsible for providing clinical and therapeutic nutrition and hydration requirements including nutritional assessments, nutritional care and follow up with residents.

Duties & Responsibilities

  • Assesses residents’ nutritional status utilizing dietary, medical and clinical data.
  • Prescribes therapeutic diets and therapeutic supplements for residents and maintains professional standards to document by utilizing electronic systems. (experience with Point Click Care preferred).
  • Educates and counsels residents, their families and caregivers on nutrition treatments.
  • Contacts and advises physicians regarding appropriate therapeutic nutrition interventions, and consideration for specific blood testing.
  • Collaborates with multi-disciplinary professionals to develop and administer effective individualized nutritional care plans.
  • Participates in case conferences regarding residents.
  • Conducts on-going communication with clients, family members and other staff, related to resident nutritional plans, referral processes, and nutritional standards.
  • Develops standards, procedures, guidelines and documentation systems for nutrition services.
  • Maintains professional knowledge and skills through continuing education.
  • Collects and analyzes quality data related to nutritional care for the purposes of quality monitoring and improvement.
  • Administers clinical nutrition program ensuring compliance with the Ministry of Health and Long Term Care Compliance Standards, including development of individual menus, and review and approval of house menus.
  • Monitors and evaluates delivery of nutrition services and dietary programs.
  • Performs other duties as assigned.

Skills & Qualifications

  • Bachelor’s Degree in Food and Nutrition or related field, along with a license with the College of Dietitians of Ontario.
  • Eligible for membership in Dietitians of Canada.
  • Working with a multidisciplinary team, the incumbent must have excellent verbal and written communication skills with strong research, time management and computer skills.
  • Highly developed analytical skills are required to interpret data and apply relevant theory principles.
  • An understanding of the Residents’ Bill of Rights and Compliance Standards as set out by the Ministry of Health and Long Term Care is required.
  • A minimum of one years’ experience as a Dietitian in a Long Term Care setting is preferred.
  • Experience with MDS-RAI and interRAI would be an asset.

  • Travel will be required; incumbent must provide their own transportation.

  • Current (obtained within the past six (6) months), original and acceptable Vulnerable Sector Check, by the first day of employment.
  • Current (obtained within the past six (6) months) two-step tuberculosis test or Quanti-FERON-TB Gold plus test by the first day of employment.
