About the Job:
Reporting to the Program Manager, the Community Mental Health Counsellor will be responsible for supporting and counselling clients experience mental health challenges in the community. The CMHC will offer strengths based case management from a recovery perspective as a part of a collaborative and interdisciplinary circle of care.
They will also be a resource and an advocate in their comprehensive understanding of crisis management and the impact mental health challenges might have on an individual and their overall wellbeing.
Their main goal is to ensure that clients with mental health challenges receive the best support possible to enable them to access and achieve independence in their home and daily activities.
Duties and Responsibilities:
- Liaise with central intake department to ensure clients smooth transition into service
- Collaborate in developing and implementing individualized wellness plans with clients
- Use standardized assessment tools designed to improve and/or stabilize client’s physical and mental health, foster independence, and promote participation and integration into the community
- Provide strength based case management that recognizes clients unique needs
- Assist with activities of daily living, conflict resolution, and crisis avoidance
- Support individuals with maintaining and sustaining housing. Participate in monthly housing meetings and/or programs dispute resolution process to support clients, as required
- Provide comprehensive, holistic, and person/family centered access to services that address the social determinants of health
- Partner with clients and their families as members of the health care team
- Develop and maintain a community resource network
- Refer to partnering organizations and provide linkages to community supports
- Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
- Understand and be able to integrate a philosophy of person and family centered care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Ensure appropriate records are kept and ensure that statistical information is maintained as per agency standards
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
Qualifications:
- Post-secondary education in Social Work, Health Services, Psychology or equivalent
- 3+ years providing Mental Health assessments and/or case management
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with intake services
- Experience working with the homeless population
- Knowledge and best evidence based practice for mental health services
- Excellent assessment, risk management, and crisis intervention
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST and CPI
Reporting Function:
The CMHC, General Mental Health will report directly to the Program Manager. They will work with both internal and external partners and will interact with all levels within the organization.
They will be a support system, resource, and an advocator for the team and Company.
Working Conditions and Physical Requirements:
- Hours are typically from Monday – Friday, 8:30am – 4:30pm but may vary depending on client needs
- Average amount of physical effort is required.
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.
