Concurrent Disorder Specialist – New Dimensions ACTT (Full Time Permanent) The Role and Opportunity The Canadian Mental Health Association is looking for a Concurrent Disorder Specialist for our Assertive Community Treatment Team – a multidisciplinary team providing treatment and rehabilitation services to individuals who have mental illnesses and are residing in Scarborough. As a member of the New Dimensions Team you will provide case management with a recovery focused approach. You must have interest in contributing to program development, experience working alongside adults with mental illness and skills in building collaborative working relationships with service providers. This position will be based out of CMHA Toronto’s Scarborough office.

Responsibilities

• Conduct outreach, comprehensive assessments, and orientation to the program and agency
• Develop individualized, evidence-based treatment plans addressing mental health and substance use needs
• Provide individual, family, and group counseling, including addictions and rehabilitation support
• Facilitate family therapy sessions to engage and educate families about concurrent disorders
• Deliver crisis intervention and ongoing therapeutic support from a recovery-based, trauma-informed perspective
• Support clients in skill development, goal planning, and accessing community resources (rehabilitation services, support groups, physical and mental health treatment)
• Provide resources, information, advocacy, and linkages to community supports
• Maintain trusting, respectful, and collaborative working relationships with clients and families
• Monitor progress, conduct evaluations, and provide follow-up care
• Participate in team meetings, provide regular updates to managers, and consult with staff on addictions-related issues
• Support agency-wide concurrent disorders initiatives, including group facilitation and staff education

Skills You Bring

• Undergraduate degree / Addictions training
• 3 years experience in community mental health and addictions
• Skills in addiction screening, assessment and counseling
• Excellent skills in case management and crisis intervention
• Implement CMHA addictions protocols
• Ability to work independently and in a dynamic team environment
• Work experience and knowledge of concurrent disorders, homelessness, recovery principles and the criminal justice system as it relates to mental health
• Knowledge and understanding of Trauma Informed Care and recovery-oriented practice
• Fluency in a second language is an asset
• Ability to work with diverse communities
• Ability to work from an anti-oppressive framework
