Job Description:

The Transitional Housing Program Coordinator is a member of the Management Team and is responsible for initiating, reviewing, evaluating and implementing program. This position will supervise all the Transitional Housing staff and the building’s front desk staff.

  • Working with the Director of Client Services to ensure leadership, coordinated development and achievement of the agency’s Transitional Housing program’s mission, vision, and strategic priorities; and the operationalization of agency values

  • Ensuring programming, policies and procedures that are consistent with Toronto Harbour Light’s mission, vision, values, and that meets strategic and operational objectives

  • Leading the agency’s team-based strategic and annual planning process and plan and prepare for program reviews and audits

  • Leading and update the agencies strategic priorities and annual goals, manage their achievement, evaluate and report goal outcomes

  • Ensuring program policies and procedures are developed, implemented and reviewed annually in accordance with THL Operating Policies Accreditation Standards and Review Procedures, legislative requirements and funding expectations, and promising practices

  • Ensure program policies are diverse and meet community and cultural needs

  • Program evaluation and developing outcomes

  • Ensuring that individualized support plans/service agreements are developed and provided for participants/clients to promote personal development and personal

  • Ensuring spiritual and religious care is available to participants/clients as requested

  • Ensuring program and agency documentation is maintained as per policies

  • Working collaboratively within our communities and continually build and maintain good relationships and/or strategic partnerships with external agencies and networks

  • Developing community partnerships which will enhance the programs and opportunities available for participants/clients and staff. Work collaboratively with other relevant service providers to create supports

  • Professional self-development to respond to agency and participant/client needs

  • Fostering cooperation, effective communication and unity of purpose between all THL programs and departments

  • Conducting case conference and other meetings, ensure minutes are recorded and circulated to team members and the Director of Client Services

  • Being aware of risk management factors and possibilities for minimizing and controlling for risks

  • Working to achieve an efficient, effective, safe and healthy work environment, and carry out responsibilities in a fiscally responsible manner

  • Leading, developing, training and supervising program staff including students to ensure work performance and maintenance of professional standards and records.

  • Providing clinical supervision including performance reviews, hiring new staff, training and orientation of new staff, ensuring all staff adhere to professional documentation

  • Working collaboratively and maintaining harmonious relations with all staff, management and the community

  • Providing ethical leadership

EDUCATION AND EXPERIENCE QUALIFICATIONS:

  • A Graduate level Social Services Related Degree or Diploma – or combination of education and experience

  • 2+ years of management/supervisor experience strongly preferred

  • Experience within housing sector preferre

  • Financial and cash and other negotiables within the social services sector an asset

  • Certification or experience in clinical supervision an asset

  • Knowledge and experience in the following areas:

  • case management

  • Concurrent Disorders and Evidence Based treatment modalities, Trauma informed service delivery models

  • Relapse prevention, Continuing Care, Crisis Intervention and Outcome Measurement

  • Knowledge of second language an asset

Compensation:

The target hiring range for this position is $66,831.48 to $83,539.35 with a maximum of $100,247.22.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
