About YOU

You are a compassionate, detail-oriented professional committed to supporting children and youth with urgent and complex needs. With a strong foundation in social services or a related health-care field, you bring at least three years of experience working with children and youth with Autism and have a solid understanding of service coordination and navigation. You are comfortable working collaboratively with families, multidisciplinary teams, and community partners to ensure seamless, family-centered care.

You excel at building strong, supportive relationships and communicating clearly—whether facilitating service coordination meetings, completing documentation, or guiding families through challenging systems. You are highly organized, able to manage a growing and complex caseload, and skilled at problem-solving with a solution-focused mindset. Your professionalism shines through in your ability to coordinate multiple services, maintain accurate records, and support families with empathy, respect, and sensitivity.

You are knowledgeable about community resources, provincial policies, and cross-sectoral collaboration, and you approach every interaction with a commitment to safety, quality, and client-centered practice. You are comfortable working in a hybrid environment, proficient with technology, and able to adapt quickly to shifting needs.

Please note that this is a contract full-time position till March 31st, 2026.

The ROLE

  • Assess eligibility for URS including appropriate level of service within the available core offerings of URS program which include service navigation, interprofessional collaboration, direct service, and respite.

  • Initiate internal and external referrals in consultation with family in collaboration with the Access and System Navigation department.

  • In collaboration with family and support team, find appropriate respite providers that match the child/youths and caregivers needs.

  • Ensure that respite tracker is updated regularly to ensure billing and invoicing accuracy

  • Scheduling and coordinating appointments in calendars of internal team, community partners and the family.

  • When required internal consultations will be coordinated alongside URS ABA team with ROCK Crisis, Psychology, Therapy, Occupational Therapy and documentation to be completed post-consult.

  • Coordinators are required to facilitate service coordination meetings with family and support team during service in accordance to the need of the family and planning, while ensuring the necessary service-providers are present to encourage community collaboration and knowledge transfer where applicable.

  • Coordinators are required to provide timely Action Plans (Meeting Minutes) to family and attendees post- service coordination meetings.

  • Documentation using ROCK client software to ensure family’s time in service is efficiently captured.

  • Maintaining calendar requirements and completing service events to reflect hours worked on weekly basis.

  • Support development, monitoring, and updating of the URS Service Plan in collaboration with the child/youth and family and their support team.

  • Facilitate the collaboration of relevant parties to refine and coordinate the delivery of the URS service elements.

  • Provide leadership and support to the development and use of an inter-agency, interprofessional and cross-sectoral URS plan that addresses the single area of urgent need for the child/youth and family.

  • Develop supportive working relationships with clients and their families.

  • A family-centered approach is prioritized when collaborating with services providers

  • Develop and maintain strong working relationships with service providers and agencies involved in client care.

  • Ensure the coordination of URS with existing services and support to promote the transfer of knowledge prior to and at the transition out of URS.

  • Work Collaboratively with other service navigation supports including IIO Care Coordinators and Coordinated Service Planners when appropriate.

  • Operationalize client safety through adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care, and by identifying and reporting any client safety incidents.

  • Commit to supporting client-centered care by adhering to ROCK’s principles of respect, collaboration, and service planning, as outlined in the agency’s strategic plan

  • Abide by the specific requirements of Section 28 of the Occupational Health and Safety Act, as well as any further health and safety responsibilities outlined in the agency's policies and procedures.

  • Other duties may reasonably be assigned.

  • You will prioritize client safety by adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care and, by identifying and reporting any client safety incidents. You are also committed to supporting client-centered care by adhering to ROCK’s principles.

Who we’re looking for:

  • A university degree in a related health care field or a Social Service/Child Youth Worker diploma.

  • A minimum of three years of experience with children and youth with

  • A minimum of two years of experience in service coordination/navigation.

  • Sound knowledge of local community and social services including but not limited to Access OAP and other Autism supports in Ontario (is an asset)

  • Strong working knowledge of provincial policies and legislation concerning children and youth.

  • Experience working in collaboration with service providers across sectors (e.g., education, health, community supports).
