About the role
This is a collaborative and integrated role between Erie Shores Healthcareand CMHA Windsor-Essex.
Reporting to the Chief Executive Officerof each organization, the Manager of Community Outreach will:
- Serve as the coordinator for the Mobile Medical Support Team and oversee the day-to-day delivery of high-quality programs and services that respond to the needs of clients.
- Act as a conduit for community engagement, strengthening partnerships and outreach efforts.
- Support vulnerable populations by facilitating access to care.
- Ensure patients are connected to primary care providers to promote continuity and better health outcomes.
Key Responsibilities
Policy Compliance & Confidentiality
- Adhere to all policies and procedures established for the assigned department.
- Maintain strict confidentiality of patient information in accordance with organizational and legal standards.
Community Outreach & Engagement
- Plan, implement and monitor services that respond to client and communities’ needs, and meet internal and external performance targets.
- Develop, implement and ensure adherence to primary care policies, protocols and workflows.
- Oversee aspects of recruitment, supervision and performance of outreach teams and maintain effective communication with employees, volunteers, patients, physicians, and the general public.
- Serve as liaison within the Primary Care Sectorand WE-OHT**,** ensuring alignment with regional priorities.
- Research and engage with community organizations that align with organizational pillars and strategic goals.
- Collaborate with the Executive Team to identify target audiences and establish partnerships with organizations serving key demographics.
- Support access to primary care by connecting unrostered patients to the Community Health Centre, the Medical Mobile Unit, or Nurse Practitioner–led clinics.
Public Engagement & Field Outreach
- Engage directly with the public in high-traffic areas to promote healthcare services, events, and partnerships.
- Observe trends and report on distribution outcomes to support data-driven outreach strategies**.**
- Provide daily or weekly activity summaries (e.g., locations covered, approximate reach, engagement notes) to track performance and optimize future planning and implementation.
- Monitor public response and engagement patterns, identifying high-performing locations and timesfor outreach.
- Support marketing and outreach campaigns by distributing branded materials that align with organizational messaging and target demographics.
- Maintain accurate logs of distributed materials and locations, ensuring alignment with outreach goals and tracking needs.
- Communicate field insights and local observations to marketing and operations teams to improve targeting and material relevance.
- Coordinate with operations and logistics staff.
Humanitarian & Partnership Coordination
- Assist in planning and coordinating logistics for humanitarian initiatives and community operations (disaster and emergencies, vaccinations, outbreak management, etc.).
- Act as liaison between local and international non-profit partners, community leaders, and healthcare stakeholders.
Executive & Strategic Support
- Provide comprehensive support to the Executive Team to ensure streamlined operations and efficiency.
- Contribute to the execution of the strategic plan and other special projects as assigned.
- Conduct research and analysis on community groups of importance to the foundation and its pillars.
- Lead and support project management activities to ensure timely and effective implementation of initiatives.
Other Duties
- Perform other relevant tasks as required to support the mission and objectives of Erie Shores Healthcareand CMHA Windsor-Essex**.**
Qualifications
- Education
- Bachelor’s degree in business administration, Public Health, Social Sciences, or a related field (required).
- Graduate-level education (e.g., master’s degree) considered an asset.
