The Opportunity: This dynamic position allows for direct patient care in a fast-paced Regional Neurology clinic. While your role will focus primarily on patients with MS, you will also provide hands-on patient care for those patients with Neuromuscular disorders, Migraine, Epilepsy and Parkinson’s. This role will be key at breaking down barriers for patient access to life-changing medicines and supporting optimal patient care. The Clinical Liaison Nurse is responsible for managing patient care, will act as a primary point of contact to their assigned health care providers (HCP) and ensures patients are provided easy access to their prescribed therapy. In addition, this role involves direct interactions with patients, insurance companies, pharmacies, physicians, and other health care professionals (HCPs) in a multi-disciplinary environment.

The ideal candidate is one who is passionate about patient care and is someone who wants to build out processes that help improve efficiencies. The candidate will embrace curiosity and be bold in building best practices that will create significant change in the way patients receive care.

A Day in the Life (What you will do here):

  • Work as a partner to the physician in a specialized Neurology clinic
  • Perform HCP and patient registration activities, according to protocols and applicable departmental procedures.
  • Provide medical support to patients and HCPs, including but not limited to education on specific diseases, therapies, and disease management.
  • Prepare for and attend extensive, specialized training sessions for specific therapies.
  • In person and virtual support (including adherence support both inbound and outbound as needed).
  • Review patient files prior to treatment to capture medical history.
  • Document and maintain patient records to ensure treatment history is adequately captured.
  • Maintains confidentiality of patient and corporate information and discusses same only with appropriate personnel.
  • Liaise with physicians’ offices, pharmacies, services providers, and other parties to provide comprehensive and coordinated service to patients and HCPs.
  • Participates in ongoing internal and/or external continuing education activities.
  • Administers medications (IM and SC) in the patient’s home or specialty clinic settings as required, as well as provides virtual oversight to remote patient groups.
  • Monitors patients post injection, and documents on all activities completed.
  • Hosts patient education based on specific patient needs, individually or in group settings.
  • Responsible for scheduling appointments.
  • Participates in telephone consultations and virtual meetings/training sessions as required.
  • Adhere to professional nursing standards at all times.
  • Work collaboratively within multidisciplinary health care team providing oversight.
  • Proactively offer support and advocacy to patients and HCP which includes but is not limited to enrollment, facilitation of coverage renewals, and changes throughout the patient journey, as well as facilitating reimbursement support to physicians and patients.
  • Provide ongoing support on disease state or product related questions.
  • Read and review all Canadian product monographs to ensure a comprehensive understanding of each product being administered.
  • Report and document Adverse Events as per training per Pharmacovigilance requirements.
  • Adhere to Sentrex policies and procedures.
  • Assist in the development and adherence to forms, policies, and work instructions, to ensure the safe handling, preparation, and administration of product.
  • Follow and abide by all provincial College of Nurses guidelines while providing patient care.
  • Other duties as assigned by your manager

What you need to ensure you are set up for success:

  • Minimum 2-4 years of clinical experience (preferred)
  • Passionate at supporting patient care and removing barriers to care
  • Licensed RPN or RN
  • Experience working in the Neurology therapeutic area is an asset
  • CPR and First-Aid certification required
  • ACLS certificate required
  • Bilingual English and French an asset but not required
  • Able to confidently deliver education including but not limited to patient self-injection education
  • Ability to confidently administer medication IM and or SC
  • Experience in a critical care, emergency, and acute care a strong asset
  • Confident and experienced with using MS Office suite: Excel, Outlook, and Word
  • Ability to work autonomously with little supervision, as part of a larger remote team
  • Experience in Patient Support Programs is a strong asset
  • Understanding of the pharmaceutical industry an asset
  • Strong analytical and mathematical skills
  • Effective interpersonal skills
  • Strong organizational skills and attention to detail
  • Ability to meet deadlines and be flexible with patient and HCP requirements
  • Excellent problem-solving skills; ability to resolve issues effectively and efficiently
  • Ability to communicate effectively both verbally and in writing
  • Adhere to Sentrex policies and procedures
