POSITION: Care Navigator

LOCATION: YWHO Chatham Site

1 Temporary Full-Time (12 month contract until May 2027)

Reporting to Manager of Integrated Client Services**,** the incumbent will be responsible for the following functions:

  • promotes a stigma free youth culture for mental illness.

  • conducting and completing the detailed Community Mental Health Assessment;.

  • assisting A/YA in prioritizing needs/areas of consideration and to develop short-term plans to address these priorities.

  • promotes opportunities for increased independence and champions community peer development.

  • acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed.

  • supporting reintegration into school, work and social activities.

Qualifications:

  • Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO, Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F.

  • 1-2 years’ experience in a health related organization and experience working with Adolescents/young adults with mental health issues;

  • Personal lived experience in recovery from a mental health and/or addiction challenges.

  • A valid Ontario Driver’s License and access to a vehicle are required.

Hours of Work: 35 hours per week, may include three days 8:30-5:00pm and one day 10:30-7:00pm and occasional Saturdays may be required to meet the position requirements.
