About the role

OVERVIEW

The Patient Transitions Flow Coordinator reports to the Manager of Integrated Care and Transitions and will be responsible for facilitating the timely transfer of patients back to their home with the MGH2HOME program. Working in collaboration with TNs, nurses, physicians and other IPP staff as required, the Patient Transitions Flow Coordinator will ensure that barriers to discharge are removed or minimized to ensure a safe and durable discharge plan is in place.

The incumbent will act as the liaison and main point of contact for TNs, patients and families while in hospital and will actively participate in the development of in home health care service plans in collaboration with MGH2HOME Community Partners. The Patient Flow Transitions Coordinator will also help to identify areas for improvement within the MGH2HOME program and processes and participate in the development of improvement strategies.

EDUCATION

  • Degree or Diploma in a health related field. (RN or RPN preferred).

  • Current certificate of registration issued by the discipline's licensing body in Ontario.

QUALIFICATIONS

  • Minimum 2 years of recent experience in an Acute Hospital setting.
  • Minimum 2 years of Health System and Community Navigation Experience.
  • Familiarity with improvement process principles is a definite asset.
  • Demonstrated resource allocation and planning experience, specifically the coordination and delivery of services.
  • Clinical Assessment skills would be an asset.
  • Knowledge of community agencies (Long Term Care, Home and Community supports etc.).
  • Demonstrated leadership skills.
  • Demonstrated computer literacy with competence in Microsoft Word, Excel, PowerPoint, Teams and Outlook.
  • Excellent written and verbal communication and interpersonal skills.
  • Proven critical thinking skills.
  • Visible, approachable and dedicated to excellence.
  • Ability to work effectively as part of a team.
  • Demonstrated ability to multi-task and problem-solve.
  • Demonstrated ability to collaborate and develop relationships with internal and external partners.
  • Excellent attendance record required.
