Horizon Health Network (Horizon) is seeking a collaborative, flexible and result-orientated individual to join our team as a Quality Patient Safety Consultant The ideal candidate will have experience in quality improvement, patient safety and human factors engineering as it relates to healthcare settings, be able to measure and analyze system, process, and outcome indicators for performance improvement, and possess strong facilitation and communication skills.

Who You Are
You have a worked in healthcare quality improvement, patient safety or healthcare operations for a minimum of 5 years. You have experience in leading improvement initiatives and working with multidisciplinary teams; measuring outcomes and evaluating the effectiveness of quality initiatives and programs; and in providing education, support and advice related to quality and patient safety.
You have strong proficiency level in Microsoft Office applications; strong systems thinking and problem-solving skills; ability to identify workflow inefficiencies and usability challenges; excellent communication, facilitation and stakeholder engagement skills; proficiency in data analysis and visualization tools (e.g. Excel, PowerBI); ability to manage multiple priorities in a dynamic environment; commitment to person-centered care, safety and equity.
Bachelor’s Degree in a healthcare related field.
Additional education or training in human factors, systems design, or related fields is an asset.
Certification in quality improvement or patient safety (e.g. Lean Six Sigma Yellow Belt, CPHQ, CPSI) is an asset.
Knowledge of healthcare standards (e.g. Accreditation Canada, CPSI, ISMP).
Travel is required, so a valid driver's license and reliable transportation is needed.
Experience leading improvement initiatives and working with multidisciplinary teams; measuring outcomes and evaluating the effectiveness of quality initiatives and programs; and in providing education, support and advice related to quality and patient safety.
Experience applying systems thinking and/pr human factors principles in healthcare settings is an asset.
What You Will Do
Quality Improvement & System Performance
Lead and support quality improvement initiatives using established methodologies (e.g., PDSA, Lean), ensuring sustainable solutions which are data-informed, practical, reliable, person-centered, and safe. Analyze performance data and frontline workflows to identify system-level trends, inefficiencies, and opportunities for improvement. Partner with clinical teams to design, test, and implement changes that simplify processes.
Patient Safety & System Learning
Promote a culture of patient safety grounded in systems thinking and continuous learning. Participate in patient safety reviews (e.g., RCA, FMEA) to identify workflow, environmental, and human contributing factors. Support the design and evaluation of initiatives that improve safety, communication, and care delivery.
Human Factors & System Design
Apply human factors principles to improve workflow, usability, and system design. Collaborate with teams to redesign processes that reduce cognitive burden, improve standardization, and align with actual practice. Promote design-based solutions (e.g., standard work, visual cues, decision supports) that enhance reliability and sustainability.
Education & Capacity Building
Deliver education and coaching around quality improvement, patient safety, and systems thinking. Support teams in applying tools and methods in ways that reflect real workflows and performance. Facilitate sharing of lessons learned to strengthen organizational learning and improvement.
Accreditation & Practice Improvement
Support accreditation and regulatory readiness by ensuring processes and practices are clear, usable, and aligned with care delivery. Provide feedback on organizational policies focusing on quality and safe patient care, considering practical application and sustainability for the organization. Support programs in meeting accreditation requirements for the organization. Lead continuous readiness activities (e.g., audits, reviews) to support ongoing improvement and learning.
Collaboration & Communication
Build strong partnerships and facilitate multidisciplinary teams to co-design practical, sustainable improvements. Engage diverse stakeholders (clinical, operational, patient) to inform system design and decision-making. Communicate insights and recommendations clearly, translating data and observations into actionable improvements.
