About the role

Job Description

Reporting to the Senior Director, Quality and Business Enablement the Clinical Practice Manager will design, evaluate, and oversee clinical programs across Extendicare homes. The Clinical Practice Manager will have in-depth knowledge and understanding of the Provincial regulatory requirements, Accreditation Canada standards, and PCC workflows to implement best practices and standard operating procedures of clinical programs in LTC homes.

This position will be located at Extendicare Head Office in Markham, Ontario. Extendicare offers a hybrid working environment.

Key Responsibilities Include:

  • Provide national oversight for one or more clinical programs and ensure compliance with Provincial Regulatory Requirements and Accreditation Canada standards
  • Review and update policies, procedures, education resources, and tools associated with clinical programs to align with regulatory requirements, Accreditation Canada, interRAI standards and best practices (as needed but at minimum once a year)
  • Review and update annual program education and orientation training material delivered to staff
  • Provide clinical program support and subject matter expertise to homes and various head office cross functional teams, including but not limited to Operations, Chief Medical Office, Compliance and Risk, and Redevelopment
  • Act as an expert in clinical workflows and tools in PointClickCare for clinical programs (e.g. User defined assessments, referrals, care plan library items, Point-of-Care tasks, etc.)
  • Lead Accreditation Canada home education and survey readiness preparation related to clinical program oversight
  • Facilitate community of practices and provide clinical program education to home program leads and regional support teams
  • Laise and collaborate with other Clinical Practice Managers to promote a standard operating model and develop a broad understanding of all clinical programs and act as a back-up for other Clinical Practice Managers as needed
  • Build and maintain clinical program competency and capacity of Regional Managers
  • Engage, lead, and support home level Program Leads related to their clinical programs
  • Guide home level Program Leads in establishing home processes to adhere to clinical program standards set out in policy and procedures
  • Conduct home visits to assess and evaluate implementation of clinical programs in homes per policy and procedures
  • Establish relationships with Extendicare contracted vendors who provide supplies, education, training and support related to their clinical program. Promote standardized service offerings across all homes for clinical programs
  • Monitor vendor service levels and contractual obligations pertaining to clinical programs. Establish escalation pathways and report on vendor related issues should any arise
  • Work with Operations and Chief Medical Office to establish program audits, health outcomes and process related KPIs for each clinical program
  • Work with Chief Medical Office Analytics and Reporting team to establish a process for monitoring and reporting on program audit results and clinical program KPIs
  • Become an expert on clinical program KPIs and program audits and educate Regional Managers and other internal cross functional teams as needed. Establish an escalation pathway when homes do not meet clinical program KPIs and program audit targets
  • Support Compliance and Risk and Operations Teams with notices and orders related to their clinical programs

Qualifications:

  • Registered Nurse or Registered Practical Nurse with a Bachelor of Science in Nursing or Diploma from an accredited nursing program
  • 3+ years of experience in an equivalent or similar role
  • In depth knowledge of PCC, Point of Care, RAI-MDS 2.0 and basic understanding of interRAI LTCF
  • Thorough knowledge of LTC legislation, program, and documentation standards
  • Demonstrated success leading new initiatives with experience in project management, coordination, and support
  • Experience with adult education strategies and facilitation
  • Proven ability to build strong relationships with individuals at all levels of an organization and external vendors and service providers
  • Ability to function effectively in ambiguity and progress projects with incomplete information
  • High degree of personal and professional integrity
  • Proficient with MS Office suite
