Qualifications
- Registered and in good standing with the College of Dietitians of Ontario
- Must provide proof of:
- Liability insurance
- TB testing
- Vulnerable sector check
- Ability to work both independently and within a multidisciplinary team
- Strong written and verbal communication skills
- Excellent organizational and time management abilities
- Demonstrated problem-solving skills
- Proficiency with computers, including experience using electronic medical records (EMR)
- Travel may be required for certain positions
Responsibilities
- Identify and monitor the level of nutrition and hydration risk for all Residents
- Complete scheduled RAI assessments: on admission, quarterly and significant status changes
- Prioritize and respond to dietary referrals in a timely manner
- Develop, evaluate, and modify individualized nutrition care plans
- Liaise with Residents, families/SDMs, and the team to implement effective dietary interventions
- Conduct swallowing assessments as required
- Provide nutrition counselling and education to Residents and their families
- Collaborate with the Nutrition Manager to enhance the dining experience
- Participate in Care Conferences and committee meetings as per organizational needs
- Review and approve seasonal menus, and create individualized menus where applicable
- Provide training and support to dietary and nursing team
- Contribute to continuous quality improvement (CQI) initiatives
- Ensure all work complies with Ontario Regulation 246/22, Section 80, under the Fixing Long-Term Care Act, 2021
- Perform additional duties as required

In the Long Term Care environment Seasons Care provides onsite Dietitian services based on MOHLTC standards, consulting, menu planning, policy and procedure development, education programs and QIP programs. In Retirement Homes we work with the homes to ensure the RHRA regulations are being met. We provide menu consultation including a comprehensive menu approval process, proactive individual consultation services, quarterly audits and site specific audits as needed. We are available to provide education sessions and full dietary management programs to our valued partners.

Directly in line with that philosophy; Seasons Care now offers Speech-Language Pathology services to ensure a Safe Swallow.
