**Position:**Personal Support Worker - Home Support and Day Program (Part-Time)
**Hours of Work:**Variable hours based on client demand
Compensation:$18.86 - $23.58
**Reports to:**Home Support Supervisor

Position Summary: Responsible to provide personal support services to enable client seniors to continue living with independence, and to be healthy and safe-at-home. Ensures satisfaction with the services by establishing and maintaining relationships with client seniors and their care providers and/or family members.

Responsibilities:

  • Provides personal care for ADLs, including bathing, washroom assistance, changing incontinence products, dressing, personal hygiene, grooming, and dental care according to the care plan.

  • Assists with the transferring and positioning of the Client.

  • Assists with meal preparation, set-up, and eating.

  • Reports all incidents to the Care Manager and/or Supervisor and follows procedures outlined in the Home Support Services Handbook.

  • Provides light housekeeping services according to the Home Making Components and the care plan.

  • Assists with program activity implementation and encourages Client participation.

  • Reports unusual occurrences (health concerns, emergencies, or service requests) to the Care Manager and/or Supervisor.

  • Reports changes in Day Program Clients’ social, physical, or mental status to the Team Leader.

  • Clocks in/out and electronically documents care plan tasks within the health record mobile app following every visit.

Qualifications:

  • Personal Support Worker Certificate

  • Valid First Aid and CPR

  • Experience working with seniors in a community program setting

  • Ability to write clear and concise client notes in English

  • Knowledge of health care, dietary needs or limitations of individuals with special needs as a result of ageing e.g. Alzheimer's, stroke, Parkinson's disease

  • Physician’s medical assessment required (A TB test form will be provided)

  • Must be available to work on evening and overnight shifts

  • Speaking Cantonese, Mandarin, Tamil, Greek is an asset

  • Vulnerable Sector screening required

  • Proof of full COVID-19 Vaccination or Medical Exemption Certificate required

Working Conditions:

  • Standing and walking for long periods of time and frequent bending
  • Exposure to agitated and confused Clients
  • May be exposed to contagious diseases
  • Required to travel to Clients’ homes; reliable transportation required for Home Support Services
  • Working alone in Clients’ homes for Home Support Services
  • Proof of full COVID-19 Vaccination or Medical Exemption Certificate required
  • Must be available at least 3 days and Saturdays
