Remuneration: $65,433.49 – $70,595.00 per annum, pro rata (+ super + 17.5% leave loading where applicable)

About The Role As a Residential Service Assistant, you will be part of a collaborative multidisciplinary Transitional Aged Care team, providing short term, goal oriented domestic and support services to older people in the community. The focus of the program is enabling clients to restore and optimise their functional capacity during a time limited intervention phase of up to 12 weeks, in a non-hospital environment.

Working under the direction of the Transitional Care Coordinator, you will contribute to the wellbeing and recovery of older people, making this a rewarding opportunity for someone passionate about delivering person-centred aged care.

Key Deliverables

  • Deliver client-enablement focused domestic services and personal care tasks that support older clients to regain and optimise functional capacity
  • Participate in the development and execution of goal-oriented, multidisciplinary care plans for all Transitional Care clients
  • Maintain accurate and timely client records in accordance with Transitional Care guidelines and health service policy
  • Contribute to quality improvement activities, accreditation processes, and statistical data collection in line with Department of Health requirements
  • Identify and report incidents and risks using the incident management system (IMS+), including work health and safety issues specific to community-based care
  • Adhere to MLHD policies and procedures and practise within the limits of your professional knowledge and skills

Our Ideal Candidate Will

  • Have demonstrated experience providing enablement-focused personal and domestic services to older clients across a variety of community settings
  • Be able to perform a full range of home duties including cleaning and basic cooking tasks to a high standard
  • Work effectively both independently and as part of a multidisciplinary team across diverse locations and services
  • Demonstrate sound knowledge of infection control principles, cleaning equipment, Department of Health cleaning standards, and Material Safety Data Sheets
  • Possess strong written and verbal communication skills with the ability to work in an organised manner and within set timeframes
  • Hold a current driver's licence and be willing to travel as required
