Post-Kidney Transplant Clinic

The Post-Kidney Transplant Clinic is seeking a Regular Full-Time Nurse Practitioner to join our dynamic team. The Nurse Practitioner is a self-directed advanced practice nurse prepared at the graduate and post-graduate level, who demonstrates a high level of autonomy working to scope, both independently and within a collaborative practice model. Using advanced assessment skills and a sound knowledge of clinical therapeutics, the NP demonstrates a comprehensive knowledge base in renal and exhibits a high degree of clinical competence in caring for post-kidney transplant patients.

The NP in the Post-Kidney Transplant Program will report to the Director of the Renal Program. The Nurse Practitioner (NP) in the Renal Transplant Program plays a pivotal role in delivering specialized outpatient care to kidney transplant recipients. This position combines clinical expertise, program leadership, and academic engagement to ensure high-quality, accessible, and patient-centered care. The NP will work both autonomously and collaboratively within an interdisciplinary team to support transplant patients throughout their post-operative journey.

The NP will interact daily with the interdisciplinary team to manage, direct and provide comprehensive care for a select patient population. The NP works with key stakeholders to provide advanced care that optimizes patient outcomes and enhances safety, effectiveness and accessibility. The NP in this program will engage in scholarly activities and advance knowledge by providing clinical leadership, identify opportunities for improving patient care, advancing best practices, promoting professional education and engaging in research.

With a focus on service excellence and patient safety, principal responsibilities include:

  • The NP will be responsible for leading and delivering care and improving access to care and decreasing wait times for this population including providing initial consultation for this population and providing appropriate and safe follow up
  • The NP will Improve access to information/education for patients and caregivers and community partners through the development of educational strategies
  • Academic Leadership: The NP will participate in collaborative research and Quality Improvement ( QI) initiatives such as: developing a program initiatives to promote patient wellness; evaluating the delivery of care to improve access to care; explore opportunities to develop Outreach clinics; patient /caregiver education; evaluation research and reporting on metrics for the purpose of program evaluation
  • The NP will collaborate with local and regional partners/stakeholders to develop, implement and evaluate best practices, standards of care and/or protocols that result in the improvement to patient care, practice and/or systems
  • The NP will plan and participate in learning opportunities for learners and participate in hospital wide in-services/education to share current evidence-based practices.
  • The NP will attend relevant conferences and professional meetings
  • Deliver comprehensive care to potential and current renal transplant patients, including assessment, diagnosis, monitoring, and management of post-transplant complications.
  • Practice to full scope in the ambulatory setting, including ordering and interpreting investigations, prescribing medications, and initiating treatment plans.
  • Lead initiatives to improve access to care, reduce wait times, and enhance patient outcomes through timely consultations and follow-up care.
  • Provide care in both in-person and virtual formats, ensuring continuity and accessibility for transplant patients.
  • Monitor long-term transplant outcomes and adjust care plans based on clinical findings and evidence-based guidelines.
  • Develop and implement educational strategies for patients, caregivers, and community partners to promote understanding of transplant care and self-management.
  • Enhance access to information through tailored resources and support tools.
  • Participate in collaborative research and quality improvement initiatives focused on transplant wellness, access to care, and service delivery.
  • Contribute to program evaluation through data collection, metric reporting, and outcome analysis.
  • Explore opportunities to develop outreach clinics and expand access to specialized transplant care.
  • Collaborate with nurses, nephrologists, transplant surgeons, pharmacists, dietitians, and social workers to deliver integrated care.
  • Engagement with local and regional stakeholders to develop, implement, and evaluate best practices, standards of care, and clinical protocols.
  • Support provincial Ontario Renal Network and Trillium Gift if Life Network priority development and implementation within the region.

Qualifications

  • Successful completion of a Master's Degree in Nursing
  • Current registration in the extended class and in good standing with the College of Nurses of Ontario
  • Minimum of 3-5 years recent related experience in renal preferred
  • Minimum of 3-5 years recent related experience in transplant preferred
  • Current member of the Nurse Practitioner's Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO) preferred
  • Mandatory Current Professional Liability Protection
  • Current Basic Life Support for Healthcare Providers course: BLS-HCP (C)
  • Valid Drivers License (G class) with the ability to travel within the South-West LHIN
  • Demonstrated experience and leadership in clinical practice, education, program development and research
  • Expertise in functioning in an inter-disciplinary health care environment
  • Ability to establish effective interpersonal relationships with colleagues and therapeutic relationships with patients and families
  • Expertise in critical thinking, clinical problem solving, negotiation and conflict resolution
  • Effective organizational, conceptual and leadership skills
  • Ability to provide project management skills to promote evidence-based strategies and initiatives for the Post-Kidney Transplant Clinic and ability to work in partnership with relevant LHSC staff and physicians
  • Experience in/or understanding research, program development and evaluation
  • Demonstrated excellence in interpersonal relations and communication skills, both oral and written
  • Demonstrated excellence as a role model for clinical leadership
  • Demonstrated experience with clinical teaching in an academic environment
  • Advanced presentation skills including the application of adult learning principles, change theory and systems theory in a clinical environment
  • Demonstrated excellence in clinical practice
  • Self-directed and function independently

  • Demonstrated skill in insight, self-awareness and self-management consistent with high emotional intelligence

  • Eligible for an Adjunct appointment to an academic institution

  • Computer proficiency with Internet, Microsoft Office 365 including Outlook, Automated Clinical Systems and Virtual meeting platforms
