With over 90,000 individuals living with diabetes in our community—and numbers expected to rise—diabetes education and care have never been more vital. At Trillium Health Partners, our Adult Diabetes Services is a multidisciplinary team that provides expert care and support to over 15,000 individuals each year through our Diabetes Management Centre (DMC) and Centre for Complex Diabetes Care (CCDC).

Centre For Complex Diabetes Care (CCDC) The CCDC offers specialized, collaborative care designed for individuals with complex diabetes needs. Our inter-professional team works together to create personalized management plans that address the unique challenges faced by patients with more complicated diabetes conditions.

Diabetes Management Centre (DMC) The DMC provides comprehensive education and management support for adults living with diabetes. We assist individuals with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and those requiring additional support during hospitalization. Our goal is to empower patients to manage their condition effectively, improving their quality of life through individualized education, resources, and care.

This Clinical Team Lead role reports directly to the Manager, Adult Diabetes Services, with accountability to the entire department, with approximately 60% of time allocated to supporting CCDC, and 40% of time allocated to supporting DMC along with the DMC Clinical Team Lead

Clinical Team Lead Responsibilities

  • Create an environment that supports and advances practice, teamwork and communication through mentoring and role modeling across the department
  • In collaboration with team and leadership team, organize and prioritize day to day operations of the team, scheduling and resource utilization
  • Participating in patient and team rounding, monitoring and addressing patient and family satisfaction with care
  • Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with department Educator and leadership team
  • Support the accountability of team members with regards to patient care, professional conduct and performance, for both clinical and clerical team members
  • Participate in the hiring, orientation and performance evaluation of staff
  • Liaise with members of the interprofessional team and regional programs to ensure excellence in care
  • Regularly complete chart reviews and audits to ensure adherence and compliance to standards of care
  • Assist in the development of effective interprofessional relationships among team through coaching, mentoring and facilitating difficult conversations to enable all team members to work within full scope of practice
  • Support patient care through a transparent, equitable and standardized process, in alignment with the Ontario Health Service Agreements
  • Participates in monitoring quality indicators including patient satisfaction and reporting requirement to Ontario Health and THP
  • Participates in the development of department specific protocol

Qualifications

  • Bachelor’s degree in a health related discipline with experience in areas of chronic disease management, diabetes management, case management or similar related experience
  • Member of a regulated health-care profession, in good standing with respective College
  • Minimum 3 years experience in a formal or informal leadership position within a healthcare environment
  • Demonstrated experience working collaboratively with interdisciplinary teams and with community partners
  • Ability to prioritize competing demands and maintain a positive work and attendance record
  • Excellent interpersonal, communication and organizational skills
  • Takes initiative to identify gaps in service and makes recommendations to address
  • Demonstrated awareness and a good understanding of the principles/protocols that govern health and safety issues
  • Commitment to professional development
  • Ability to problem solve clinical issues with the support of the manager
  • Possess the ability to implement strategies that effectively facilitate change
  • Excellent computer skills i.e. Meditech, Microsoft Office, EPIC and other clinical applications
