Social Worker (Full-Time, Shared Role – Vancouver Island)

Cerwydden Care & Oyster Harbour Care Centre

Park Place Seniors Living is seeking a compassionate and experienced full-time Social Worker to support residents, families, and care teams across two beautiful Vancouver Island communities.

This unique shared role offers variety, connection, and meaningful impact:

  • 2 days per week at Cerwydden Care
  • 3 days per week at Oyster Harbour Care Centre

About the Role

As the Social Worker, you will play a key role in supporting residents’ psychosocial well-being while partnering closely with families, clinical teams, and external agencies. This role is ideal for someone who values relationship-building, advocacy, and continuity of care across multiple settings.

Key Responsibilities

  • Provide psychosocial assessments, counselling, and emotional support to residents and families
  • Support residents during transitions, health changes, and end‑of‑life care
  • Assist with care conferences, care planning, and interdisciplinary collaboration
  • Provide guidance on community resources, financial supports, and advance care planning
  • Support admission, discharge, and adjustment processes
  • Act as an advocate for residents’ rights, dignity, and quality of life
  • Collaborate with site leadership and care teams to address concerns and complex situations

What You Bring

  • Degree in Social Work (BSW or MSW)
  • Registration or eligibility for registration with the BC College of Social Workers
  • Experience in seniors care, long‑term care, health care, or a related setting (preferred)
  • Strong communication, empathy, and relationship‑building skills
  • Confidence working independently while collaborating with multidisciplinary teams
  • Valid driver’s license and ability to travel between sites
