Summary Of Duties Reporting to the Paramedic Manager, Mobile Integrated Health Program, the Health Outreach Coordinator will lead and coordinate the activities of 20+ health outreach agencies in Hamilton, serving populations impacted by mental health challenges, addiction, homelessness, and encampment living. This position will centralize interagency communication, streamline logistics, and ensure service delivery aligns with the Greater Hamilton Health Network’s (GHHN) goals and the City of Hamilton’s Housing Services Division.
The position will work with City staff and community partners in a multi-functional initiative in support of the coordination and delivery of integrating outreach efforts. This position works independently, with minimal or no supervision, on multiple priorities and ensures deadlines are adhered to and established procedures followed.
GENERAL DUTIES
Interagency Coordination and Communication
- Develop and maintain partnerships between and among all health outreach groups, community groups and non-profits.
- Coordinate activities and logistics across all health outreach programs, supporting efficient service delivery in partnership with community groups, and non-profits.
- Serve as the central point of contact for inquiries regarding health outreach services from agencies, staff, the public, and elected officials. Respond to these inquiries promptly, following program guidelines and established policies.
- Implement interagency protocols for health outreach, dispatching, partnership brokering, and program monitoring, ensuring alignment with provincial guidelines and City commitments.
Stakeholder Engagement and Event Coordination
- Assist with consultation and event coordination to engage divisional partners, external stakeholders, and people with lived experience, ensuring diverse perspectives are included in program planning and improvement.
- Promote community and stakeholder engagement through regular public forums and feedback collection to refine and adapt services based on direct input from service users and partners.
Policy Development and Program Oversight
- Develop and oversee health outreach policies and standards for service delivery in alignment with best practices and City goals. Ensure health outreach activities meet quality assurance standards, city-wide commitments, and GHHN’s integrated care objectives.
- Review and monitor health outreach activities to ensure alignment with commitments to Hamilton City Council, divisional partners, and the City’s strategic mission.
Data Management and Reporting
- Compile and organize data from diverse sources for monitoring program effectiveness, client outcomes, and quality improvement.
- Implement data-driven strategies for continuous improvement, regularly reporting findings to the Director of Community Services and other stakeholders.
Resource Optimization and Training
- Organize training sessions on trauma-informed care, harm reduction, and cultural competency to build capacity across health outreach staff and agencies.
- Oversee resource-sharing arrangements and support joint funding initiatives, optimizing the use of resources for community impact.
- Assist in developing new standards for health outreach work, applying best practices to exceed service standards and lead by example in operational improvements.
Qualifications
- College diploma in public administration, social policy/planning, social work, or related program with the demonstrated relevant experience related to the duties described or an equivalent combination of education and relevant work experience.
- Considerable experience in mental health, addiction, homelessness, harm reduction, and outreach services; familiarity with integrated service models and local/regional outreach policies.
- Knowledge of outreach-specific practices and trends, including reviewing, recommending, and implementing operational policies and procedures.
- Highly effective leadership, facilitation, communication, presentation, interpersonal and organizational skills.
- Ability to work under pressure to meet tight externally and internally driven deadlines.
- Possess excellent verbal and written communication skills.
- Demonstrated ability to make decisions involving multiple routine tasks. Demonstrated ability to work independently.
- Possess a demonstrated record of customer focused service delivery, self-motivation, and a commitment to results and continuous improvement.
- Experience in the housing and homelessness sector, mental health, addiction, harm reduction and outreach services, along with familiarity with integrated service models and local outreach policies.
- Strong computer skills. Must possess thorough knowledge of the current suite of Microsoft productivity tools, including PowerPoint, Excel, and Word.
- Possess a demonstrated record of leadership and guidance, technical competence, customer focus, innovation/creativity, teamwork, staff delegation and empowerment, and be committed to results.
- Ability to effectively work with a large multi-disciplinary staff in a results-oriented environment and in a predominantly unionized environment.
- Ability to deal effectively with representatives of other levels of government, management, peers, staff, advocacy groups and the general public.
- Class G Driver’s license an asset.

The incumbent(s) or the department has the option to have the salary grade assigned to this position re-evaluated within nine months of the initial filling of this position. The Director of the division must review and submit the submission to Human Resources within 45 days of receiving it. All required forms of the job evaluation submission must be fully completed in order for the review to occur. If fully completed and updated job evaluation forms (with an updated draft job description) are not received within nine months of the initial filling of the position, the existing job description and rating shall be confirmed.
