Job Description

JOB SUMMARY

The Public Health Nurse (PHN) works within a multi-disciplinary team to provide services in health promotion, health protection, and disease treatment and prevention for individuals, families, groups and communities, under the direction of the Senior Leadership Team of Niagara Region Public Health and Emergency Services (NRPH & EMS). The PHN combines knowledge from public health, social and nursing sciences to engage in promoting health, assessment, planning, implementation and evaluation of public health interventions and services. Services are provided virtually and in person through home or office visits, community settings and presentations, agency liaison and collaboration.

The PHN is guided by departmental priorities, the Ontario Public Health Standards, the Canadian Public Health Association competencies, Community Health Nursing standards and the College of Nurses of Ontario standards.

The PHN works according to the College of Nurses of Ontario’s Practice Standards and program specific plans, policies, procedures and guidelines, while adhering to corporate and departmental policies.

QUALIFICATIONS

EDUCATION

  • Baccalaureate degree required in Nursing (BScN/BN)
  • Current Registration as a Registered Nurse with the College of Nurses of Ontario
  • Current certificate in CPR(HCP/BLS)

KNOWLEDGE/EXPERIENCE

  • Experience in a community health or public health setting is an asset
  • Experience working with client populations as defined under the specific program requirements
  • Knowledge of infection prevention and control interventions

SKILLS

  • Past accomplishments will demonstrate excellent communication skills (verbal and written), group presentation and facilitation skills, counseling skills and the ability to work effectively on a multidisciplinary team
  • Demonstrated knowledge of the social determinants of health and experience working with vulnerable populations
  • Demonstrated proficiency in competencies of community health nursing
  • Demonstrated ability to participate and contribute to shared health promotion program plans created

in multidisciplinary teams

  • Skills in vaccine administration
  • Computer literacy is required
  • Fluency (written and spoken) in French and other languages is considered an asset

SPECIAL CONDITIONS

  • Work flexible hours including weekend, standby (on-call) and/or evening hours
  • Vaccinations as outlined in departmental policy
  • May be redeployed to nursing work across the Department as required
  • Must provide own transportation to be able to travel throughout the Niagara region and must possess and maintain a valid driver’s license to the class of vehicle operated

PHN JD Language

Infection Prevention and Control (IPAC) Program

PHNs that are a part of the Environmental Health team will work directly with the IPAC Hub program Team Leader and Manager. The Team Leader and Manager act as the regional (HNHB) and provincial points of contact.

70% of time will be focused on IPAC duties including:

  • Manages cases, conducts contact tracing, and supports individuals with reportable infectious diseases while participating in investigations in both regulated and unregulated healthcare settings.
  • Provides consultation and support during outbreaks for infection control and policy development to
  • healthcare professionals and various care facilities, including childcare and schools.
  • Conducts investigations for enteric diseases and food/waterborne illnesses, including sample collection, and participates in community outbreak investigations.
  • Delivers educational presentations on infection control practices to healthcare facilities and partners including at CME credited events.
  • Contributes to the development of program policies and procedures by ensuring the application of evidence-based practice as guided by the Ministry of Health and Long-Term Care, the Ontario Chief Medical Officer of Health’s Office, Public Health Ontario, and the Public Health Agency of Canada.

30% of time will be focused on IPAC Hub (Congregate Living Settings) duties including

  • Educate, inform, and support leaders and staff in congregate living settings to respond to identified gaps in infection prevention and control (IPAC) practices, and collaborate on developing tailored strategies to address those gaps across diverse environments.
  • Apply and translate IPAC best practice guidance to address setting-specific needs as well as sector-wide priorities, ensuring guidance is relevant, accessible, and actionable.
  • Implement proactive, consultative supports designed to strengthen preparedness and resilience within settings prior to an outbreak, including coaching, resource-sharing, and capacity-building activities.
  • Liaise with Public Health colleagues to ensure consistent communication regarding IPAC best practice updates, emerging issues, and progress made within local congregate living settings.
  • Support continuous quality improvement by assisting settings in integrating sustainable IPAC practices and monitoring adherence to recommended standards.
  • Foster collaborative relationships with partners to enhance coordinated IPAC planning
  • Completion of Infection Prevention and Control course and certification by the Certification Board of Infection Control and Epidemiology Inc. is preferred.
