Located within the Rocky Mountain House Health Centre, this role offers the opportunity to work with a collaborative interdisciplinary team while making a meaningful impact in the lives of people in your community. Rocky Mountain House is located 77 km west of Red Deer in west central Alberta along the North Saskatchewan River. The community offers a welcoming small town atmosphere with access to modern amenities and a dedicated welcoming committee that supports newcomers in getting connected. As a Licensed Practical Nurse, you will report to the Home Care Manager and work closely with the Registered Nurse Case Manager to assess, plan, monitor, and evaluate client care. This position includes a strong leadership component and allows you to make a measurable impact on service delivery. You will coordinate and provide client centered care in home and alternative care settings, working with clients and families to support safe and individualized care plans. In addition to direct care, you will support operational needs by ensuring schedules are appropriate and aligned with staffing requirements. You will identify and communicate staffing pressures, support LPN team members, and act as a liaison between frontline staff and leadership. Success in this role requires adaptability, strong organizational skills, and the ability to manage a high volume workload through prioritization and delegation. You will demonstrate sound decision making, apply conflict resolution skills, and lead through accountability and professionalism. Strong time management and the ability to work in varied home environments are essential.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Candidates must possess a valid driver’s license and have consistent access to a reliable vehicle suitable for community travel. A proven track record of reliable attendance, dependability, and professional conduct is essential and will be verified through employment references. One (1) year of demonstrated leadership experience, along with prior case management experience. A minimum of one (1) year of wound care experience is also required, as wound management constitutes a significant portion of this role.
Preferred Qualifications:
Experience providing case management with knowledge of Connect Care and service authorizations. Skilled in Home Parenteral Therapy, wound care, lower limb assessments, skin and ostomy care, palliative support, and client teaching. Knowledge of Continuing Care Health Service Standards. Minimum two years of recent Home Care experience or relevant community healthcare experience with strong clinical and care coordination skills.
