Job Summary:
Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Ontario Health regions’ Behaviour Support Action Plan. A key component of the plan is the deployment of a Behaviour Support Outreach Team (‘BSOT’), consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise. The nurse will travel to various community locations and long-term care (LTC) homes in the Toronto Ontario Health region and work with the inter-professional team to provide collaborative and evidence-based care. *This position will be predominantly working in the east end of the Toronto Ontario Health region.
Responsibilities include but are not limited to:

Provides sound clinical knowledge and skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge

Plans for, delivers and evaluates non-pharmacological behavioural interventions for clients with dementia and/or mental health conditions
-
Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of a clients
-
Assists staff in the Toronto Ontario Health LTC facilities to understand and develop behaviour approaches to providing care to residents requiring behavioural support and their families.

Collaboratively develops and plans assignments with PSWs, community agencies and LTC Teams
-
Participates in team meetings and contributes to team development
-
Participates in data collection and reporting on a number of accountability measures
-
Performs teaching responsibilities, participates in relevant education and research activities, and performs duties consistent with the job classification, as required.

Performs departmental responsibilities to contribute to the efficient and effective provision of service.

Ensures the protection of client rights and acting in an advocacy role for the resident/clients.
Qualifications include but are not limited to:
-
Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
-
Four (4) year B.Sc. Nursing Degree required
-
Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
-
Access to a reliable transportation is required due to regular travels in the community
-
Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients and families.
-
Minimum 1-year related experience in geriatrics, community based seniors care and behavioural management; three (3) years preferred
-
Experience working in the LTC and community sectors preferred
-
Training in Gentle Persuasive Approach, DementiAbility and/or PIECES preferred
-
Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
-
Demonstrated ability to coach and mentor others
-
Ability to travel to different locations
-
Excellent interpersonal, communication and problem-solving skills
-
Basic computer skills – Meditech, MDS/RAI, Microsoft Word and Excel preferred
