Job Description POSITION PURPOSE SUMMARY: The Intake Case Worker supports, assists, advocates for and refers clients/families in need. The incumbent ensures that clients understand the process of accessing services and help empower them to become more self-sufficient, while also assisting with immediate practical needs.

Accountabilities Client Services and Programs

  • Conduct intake services, client assessments and emergency assistance (including food, clothing and household needs as available) in accordance with The Salvation Army’s policies, procedures, and best practices.
  • Create a safe space for clients where they can explore their practical, social, emotional and spiritual needs.
  • Assist individuals in determining appropriate and mutually agreed upon goals; facilitates effective problem-solving.
  • Ensure that the ministry objectives are accomplished, and that spiritual focus is applied in all programs.
  • Refer clients to the Corps Officer for pastoral assistance as needed.
  • Assist clients with LEAP and OESP applications.
  • Should Pathway of Hope be implemented, perform the duties associated with the case manager role under the Pathway of Hope framework including but not limited to initial intake and assessment, goal setting, linkage to appropriate services and supports and goal assessment in cooperation with the POH participants.
  • Periodically review program assistance guidelines with supervisor in accordance with TSA standards/guidelines.
  • Work with other CS staff to maintain, promote and improve the profile of TSA Chatham-Kent Ministries Community Church and CS programs within the community; actively promote Chatham-Kent Ministries Community Church and CS programs to clients.
  • Network with referral agencies and/or other community sources as required.
  • Refer clients to other community resources where appropriate.
  • May assist with other food bank operations, or CS programming when needed.
  • Maintain food bank stock, cleanliness, tidiness, and organization of food bank area including but not limited to sorting/storing/organizing all incoming donations and food rescues and ensuring products get distributed before expiration.
  • Implement/organize CS programs that include a spiritual and holistic dynamic in coordination with the Corps Officers.
  • Participate/partake in CS programs as directed by Corps Officers.
  • Through conversation and prayer, share the hope and love of Jesus in a manner that supports and respects the faith journey of our clients and their families.
  • Network with TSA, government, and community social services providers to coordinate resources for the benefit of clients and programs.

Seasonal / Special Programs

  • Assist in the organization, planning and delivery of seasonal and special programs such as Christmas assistance, kettles, food drives, backpacks, community lunch, anger management and other CS programs as required.
  • May be required to pick up community donations of food etc.
  • Participate, support, and/or attend fundraisers done for The Salvation Army by various community partners.

Volunteers

  • As directed by Corps Officer, participate and/or assist in recruitment, orientation, training, and supervising of volunteers.
  • Oversee day-to-day volunteer activities.
  • Demonstrate work methods, provide supervision and logistical guidance, and ensure that they work together to achieve the purpose of the organization and enhance the ministries of The Salvation Army.
  • Monitor the performance of all volunteers to ensure high quality services are provided to our clients and other departments.

Administration

  • Prepare and maintain accurate and complete case notes and participant records.
  • Utilize standardized assessment tools including pre-screening, intake and selection assessment and evaluation and develop program plans based on participant’s needs. Evaluate the participant’s progress toward program goals.
  • Maintain accurate L2F data reports as required; accurate records of client information on the appropriate Salvation Army client databases; ensure documentation reflects services rendered using all appropriate forms, and detailed case notes for every Pathway of Hope interaction.
  • Ensure confidentiality is maintained in accordance with Operating Policy and applicable legislation.

Health and Safety

  • Assist in ensuring that the Community Services areas are maintained regarding appearance, condition and safety as recommended by Public Health
  • Work in compliance with the OH&S Act and Regulations and abide by TSA's health and safety policies and procedures.

Working Conditions Perform other related duties as required.

  • This is a regular full-time position based on 37.5 hours per week.
  • The performance of the job requires the ability to lift/move 30 lbs., reach, bend, twist, and stoop frequently, work in a standing or sitting position for long periods, un-packing delivery vehicles, and computer keyboarding.
  • Exposure to scents and fumes.
  • May be exposed to disagreeable people including the occasional angry and/or abusive client/public, working with people from all walks of life and backgrounds.
  • Some travel up to 5% will be required and may include may be required to travel to meetings outside the immediate area.

Normal hours of work : 8:30 a.m. to 4:00 p.m. Monday to Friday and includes a ½ hour paid meal break. Flexibility in scheduling is required as some weekend and evening work may be required.

PLEASE NOTE: it will require greater scheduling flexibility during the months of November and December due to donation pickup/distribution for seasonal programming.

The above responsibilities must be performed in keeping with The Salvation Army’s Mission, Vision, and Values, in a professional manner, upholding our code of conduct. Education, Experience, Qualifications, Certifications, Skills Education, Qualifications and Certifications:

  • Completion of a two (2) year Community College Diploma in a human service field, i.e., Social Services Worker, Community Services Worker, Child, and Youth Worker.
  • A University Degree in a relevant field of study would be a definite asset, i.e., Bachelor of Social Work, or Bachelor of Arts in a social services field such as Psychology, Sociology etc.
  • Ability and willingness to provide or obtain valid Safe Food Handlers, First Aid/CPR and Non-Violent Crisis Intervention Certifications.
  • Valid Ontario Class G driver’s license, own vehicle and insurance, provide a driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, is required.

Experience And Knowledge Requirements

  • A minimum of two (2) years of prior related experience in providing casework services, documenting, and maintaining case files, working collaboratively with other service providers, community outreach, and working with people of diverse backgrounds.
  • Have a keen interest in the social services sector and a desire to see lives transformed. Possess a genuine concern for and interest in the marginalization of society.
  • Demonstrated knowledge and prior experience with motivational interviewing and strengths-based case management.
  • Proficient in a computerized environment; must have strong skills using Microsoft Office Suite.
  • Hands-on experience and human services experience working with marginalized individuals is required. A working knowledge of mental health and addiction issues.
  • Strong knowledge of community-based resources is required and experience networking with government and social service agencies.

Compensation The target hiring range for this position is $21.83 to $27.29 with a maximum of $32.75.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
