About the role
Full-Time Term (37.5 hours/week as best suits the program)
Responsibilities: Supporting individuals & families in securing and maintaining safe, stable housing. This role connects clients with appropriate housing options, resources, and supports, while fostering positive relationships with landlords, service providers, and community partners. The Housing Coordinator works collaboratively with other staff at the Community Connections Centre to promote housing stability and overall well-being.
Qualifications:
- Valid Class 5 Driver's Licence;
- Must Pass a Criminal Record, Vulnerable Sector check;
- Ability to work with diverse groups of people in a collaborative, culturally aware manner;
- Knowledge of the immigrant/refugee settlement and integration process;
- Knowledge of chronically homeless population & understanding the principles & practices of Trauma Informed Care;
- Mental Health First Aid/willingness to get;
- Understanding and awareness of First Nations Culture;
- Good written and oral communications skills;
- Ability to work independently & as part of a team;
- Demonstrate a pro-active approach, flexibility, tolerance, perseverance, and competence;
- Post-secondary education partial university or other relevant training & experience in the area of Mental Health/Addiction recovery;
- Experience in working with people with mental health, substance use, and/or cognitive disabilities;
- Excellent time management skills;
Duties & Responsibilities:
- Support clients in finding, securing, and transitioning into safe, appropriate, and affordable housing in and around the community;
- Develop individualized housing plans in collaboration with clients based on their unique needs and goals;
- Provide ongoing, culturally sensitive case management support to help clients sustain their housing including life skills, budgeting, and tenant rights education;
- Build and maintain strong relationships with local landlords, housing providers, and community partners;
- Identify and address barriers to stable housing, including mental health, substance use, income instability or legal issues;
- Referring clients to budget counselling and supports to build sustainable financial skills;
- Working with Coordinated Access and HIFIS;
- Connecting participants to other community services;
- Maintain accurate and up-to-date documentation, including client files, housing plans, and progress reports;
- Attend housing/homelessness related meetings and committees as needed.
- Assist clients with Identification by connecting to resources;
- Collaborate with internal teams and external agencies to ensure a wraparound approach to care and support;
- Ensure critical incidents are properly documented and reported to management;
- Outreach: Grocery shopping, follow up visits, assistance moving, house viewing, transportation to appointments, etc.
- Uphold and maintain policies and procedures for all programs;
