WAGE RATE: $37.071 – $38.157
Reporting to the Manager of the Crisis Team, the Registered Practical Nurse will provide comprehensive mental health assessment and case coordination service to persons that are experiencing a psychosocial crisis or psychiatric emergency, have been recently discharged from the inpatient MH unit or being connected to the Urgent Clinic. This incumbent will have a thorough knowledge and understanding of community resources, family, hospital and social systems to collaborate with team members on appropriate responses and intervention to support individuals and their families. This position is initially targeted to work with our Community Crisis Team with the understanding that the RPN is a member of Outpatient Mental Health Services and as such may be transferred to another program as needed.
QUALIFICATIONS
Required:
- Current Registration with the College of Nurses of Ontario
- Certificate in Psychiatric Nursing from a recognized Community College
- Experience in working with individuals in a psychosocial crisis or psychiatric emergency and working with community agencies
- 3 years working with persons with a Serious Mental Illness
- Psycho/social and mental status assessment skills
- Current CPI Certification or willingness to take training
- Working knowledge of Mental Health Act, Health Care and Consent Act and Substitute Decision Makers Act, Child and Family Services ACT, DSM- IV (R) is essential
- Ability to communicate positively and professionally with family members, community providers and physicians utilizing telecommunications systems and face to face skills.
- Ability to respond and prioritize multiple departmental requests to provide quality client centered care
- Ability to problem solve and make effective decisions in a fast paced, stressful emergent environment
- Ability to engage patients using knowledge and skills related to motivational interviewing, cognitive behavioural and brief solution focused therapies
- Ability to work collaboratively with the multidisciplinary team, patient and /or family to develop care planning for patients with complex treatment needs
- Recent satisfactory performance and attendance records
