The Supportive Housing- Addictions & Mental health Worker will perform duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of individuals in the Middlesex Accommodation Program (MAP) and work with other Supportive Housing team members in Strathroy and Middlesex to support the needs of clients within our continuum of Supportive Housing. This staff will support individuals who experience persistent and chronic homelessness with a focus on supporting them to achieve housing stability. This role works collaboratively with the Middlesex County Library Community Navigator and the Middlesex Accommodation Program (MAP).

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ( $30.01 per hour for Level 1 – Direct Service, Regulated):
    • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated); for internal applicants, grid level placement will occur as per the Collective Agreement
  • When you'll work - 70 hours over two-week period, Monday to Friday 8:30am – 4:30pm
  • What your benefits will be - 9% in lieu of benefits
  • Your pension plan – Immediately eligible for optional enrolment in Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:

Primary staff for Middlesex Accommodation Program (MAP)

  • Assess referrals for program eligibility
  • Ensure individuals’ needs are being met and that each individual is supported to maintain housing
  • Provide community outreach, information and referrals to individuals who are experiencing homelessness and who may benefit from services
  • Ensure individuals’ Electronic record and paper file is complete and up-to-date
  • Provide transitional support and warm transfer to new resources/housing
  • Provide updates on individuals-served at Team Meetings
  • Work collaboratively with community partners to navigate and locate resources

Direct Service

  • Support individuals to secure housing opportunities
  • Provide intensive support including regular, continuous contact with the individual while residing at the MAP
  • Support Motel Management as necessary when concerns arise
  • Motivate and support individuals to work towards achievement of their needs and/or goals
  • Advocate, refer and connect individuals with appropriate services, supports, and resources (formal and informal) based on their needs.
  • Support individuals with physical and mental health and addictions concerns, liaise with clinical staff
  • Support and bring awareness to recreational community outings and activities to enhance individuals’ quality of life and further connect individuals to their community
  • Assist individuals to successfully remain in the program, providing prevention and diversion strategies to avoid homelessness
  • Works in collaboration with the greater Supportive Housing Team that supports the presenting needs of clients in Strathroy and the county of Middlesex

Assessment and Goal Planning

  • Assess individuals’ strengths, abilities, needs and preferences
  • Assess social and environmental factors impacting the individual, and any vulnerability and risk factors before admission into the program.
  • Include and encourage individuals’ natural supports (e.g. family, significant friends) in goal planning, clarifying the roles each plays towards each individual’s rehabilitation as appropriate
  • Regularly solicit feedback from the individual-served and relevant community supports regarding the individual’s mental health and progress towards his/her goals

Communication and Decision Making

  • Make decisions both independently and collaboratively
  • Complete required individual documentation as per CMHA TV standards
  • Complete other required documentation (e.g. monthly time sheets, vacation requests, travel claims, expense claims) within the required time frames
  • Provide input into decisions with the Manager concerning operations of the program
  • Ensure effective communication with Director, Managers, Team Lead.
  • Identify, promote resolution, and manage emergency situations concerning individuals, staff, and property
  • Knowledge of issues pertaining to individual confidentiality and consent to release personal information to ensure individual’s confidentiality is maintained at all times Other duties as required

WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. We require the following qualifications:

  • Diploma or Degree in the social science field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Valid Driver’s licence and access to a vehicle with at least 2 million in liability insurance for occasional transportation of clients
  • Clear, current Police Vulnerable Sector Check (PVSC)
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • 1 year of work experience in the mental health field
  • Experience working with homeless/marginalized populations
  • Knowledge of Psychosocial Rehabilitation and Harm Reduction model
  • Residential experience
  • Knowledge of psychiatric medications
  • Knowledge of community resources
  • Knowledge of computerized documentation methods
  • Health and safety knowledge
  • Crisis intervention and prevention
  • Applied Suicide Interventions Skills and Training
