Hours: 37.5 Hours, Weekly

Status: Permanent Full-Time

Closing Date : Until Filled

To provide professional social work services in an Outpatient Stroke/ABI Rehabilitation Department to patients and families towards improving quality of life, given changes in their social, emotional, psychological and physical situation.

  • Provide social work services including assessment, counselling, advocacy & location of community resources and discharge planning for clients and families.
  • Development, planning, implementation and evaluation of social work services as required.
  • Facilitate education and support groups for clients and families
  • Participate in related in-service education.
  • Assist with discharge and transition process, regularly assessing needs such as psycho-social support for the patient and family as well as applying for funding support while meeting length of stay targets.
  • Complete clinical workload statistics.
  • Pursue continuous professional development.
  • Work closely with team members in handling situations requiring counselling and support around care, treatment, and decision making as well as any applications (e.g. funding, LTCH, etc.) that are required.
  • Ensure timely and appropriate clinical documentation
  • Develop strong relationships community providers to facilitate partnerships between patients, family, inpatient clinical team and referring agencies (e.g. LHIN, referral sources, etc.).
  • Provide resource clinical leadership for the inter-professional team.
  • Provide leadership in program evaluation, program development and research as appropriate.
  • Participate in internal and external committees.
  • Partner with Service Managers to ensure clinical service goals are addressed.
  • Ensure that psycho-social issues / concerns are addressed in a timely and effective manner.
  • Ensure that care is patient-focused with an emphasis on culturally appropriate care that keeps equity, diversity and inclusion in mind.
  • Perform other duties as assigned.
  • Be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors including:
  • Compliance with the safety standards of the hospital and applicable legislation
  • Ability to work in a manner that exemplifies the core values of West Park: Excellence, Respect, Trust, Collaboration and Accountability
  • Evidence of good performance history and attendance record

Qualifications Education: Required (Minimum):

  • Master’s degree in Social Work or Baccalaureate degree

Desired/Preferred:

  • Member of relevant professional association/network preferred

Accreditation/Certification/Designation or license through a professional body or association, i.e. RN, CHRP, CPP, etc.: Required (Minimum):

  • Membership in Ontario College of Social Workers and Social Service Workers.
  • Must be able to achieve 80% in the Functional Independent Measure (FIM) certification exam within the probationary period and on recertification exams if applying to work on any of the following rehabilitation units: 3EA (Musculoskeletal Rehabilitation), 3EB (Amputee Rehabilitation), 3EC (Neurological Rehabilitation), 2EB (TB Rehabilitation) and 2WF (Respiratory Rehabilitation).

Desired/Preferred:

  • Fluency in a language other than English an asset
  • Experience in a hospital setting

Required (Minimum): Experience (number of years of job-related working experience) :

  • Ideally, minimum of 1 - 2 years clinical experience
  • Knowledge of and preferred experience with Outpatient Neuro
  • Demonstrated leadership abilities in the following areas: co-ordinating internal & external resources; community liaison; education; advocacy, training & support to varied professionals
  • Strong assessment skills
  • Knowledge of a variety of community resources
  • Demonstrated experience in discharge planning and transitions implementation
  • Strong interpersonal, organizational and communication skills (both written and verbal)
  • Demonstrated ability to partner and work effectively with all levels of staff, the interprofessional team, physicians, patients, families and external agencies
  • Demonstrated ability to deal with complex mental health and behavioral issues
  • Ability to maintain effectiveness in varying environments and with different tasks, responsibilities and people
  • Demonstrated knowledge of and ability to provide patient centred care.
  • Demonstrated problem solving, interpersonal, coaching, negotiation and facilitation skills
  • Strong organizational skills to work independently in an unstructured, demanding environment
  • Demonstrated reliable work habits
  • Attendance at safety meetings/training/updates in accordance with the Centre’s standards
  • Availability to work a flexible work schedule (some evenings are required)
