The Opportunity:

We are recruiting for an ADMINISTRATIVE AND YOUTH FACILITATOR (The Grove – Youth Wellness Hubs Ontario)

There are two opportunities available:

  • One regular full-time opportunity (35 hours per week Monday to Friday 12pm to 8pm) based at 737 Woolwich St. Guelph.
  • One regular full-time opportunity (35 hours per week Monday to Friday 12pm to 8pm) based at 900 Tower St. S. Fergus.

The Administrative and Youth Facilitator conducts wellness services intakes for youth, families, service providers and community members reaching out to access clinical services and gather more information about the Grove Youth Wellness Hub Ontario. The Administrative and Youth Facilitator will work as a member of a diverse interdisciplinary team to support youth and their families accessing YWHO services. This role will support a consistent and centralized intake process to all services available through the Grove Youth Wellness Hub Ontario by championing the use of YWHO’s electronic client information system and screening tools. The Administrative and Youth Facilitator will provide services reflective of the diverse needs of the youth and their families. This position reports to the Team Lead, The Grove.

RESPONSIBILITIES

  • Oversee the intake process for youth and families accessing wellness services, including the use of YWHO’s electronic client information system and appropriate screening tools, and the use of CaseWorks to plan, document and gather feedback on all activities.
  • Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe, and welcoming hub atmosphere.
  • Conduct intakes for wellness services with youth and families, including screening tools within My Wellness Passport, and the use of CaseWorks to plan, document and gather feedback on all activities.
  • Collaborate with community partners for skills and wellbeing programming
  • Represent YWHO to youth, families, and service providers, and support an orientation to the hub and an introduction to YWHO’s electronic client information system and measurement-based care.

QUALIFICATIONS:

  • Diploma or degree in social services or a combination of education and experience that will meet the needs of this position
  • Minimum 1 years' experience working with children and youth in community based mental health/substance use
  • Experience with health information systems is an asset
  • Knowledge of community resources available to support youth and families across a continuum of care and wholistic understanding of health and mental health
  • Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks
  • Excellent organizational, and written communication skills
  • Excellent interpersonal skills in connecting/building relationships with youth, family/caregivers and service partners
  • The ability to work effectively as a team member in a dynamic and fast-paced multidisciplinary environment is essential
  • Experience in supporting safe and meaningful participation and being open to diverse ways of thinking about, understanding and articulating challenges related to mental health and substance use is required
  • Training in ASIST and PreVenture an asset
  • Proficient with MS Office and data based software
  • A valid driver's license, reliable vehicle and insurance as travel is required

COMPENSATION AND RATE OF PAY: Level 3 (OPSEU) - $25.83 to $31.38 per hour
