Murrumbidgee Local Health District is looking for a qualified healthcare professional to assess aged care needs and facilitate access to appropriate support services for older people across the region.
About The Role The Clinical Needs Assessor utilises assessment tools to determine care needs for frail older people and facilitates access to appropriate aged care services. This role takes a wellness-focused approach to maximise client independence while meeting Commonwealth performance standards.
Responsibilities
- Assess care needs using the Integrated Assessment Tool and coordinate access to appropriate aged care services
- Apply a wellness, reablement and restorative care approach to maximize client independence and quality of life
- Review and adjust care plans as client needs change, enabling informed decision-making about their care and services
- For qualified RAC Aged Care Assessors: assess residential aged care funding needs using the AN-ACC tool, completing 4-6 assessments per day depending on travel requirements
Employment Type: Temporary Part Time (until 30 May 2027)
Position Classification: Dependent upon qualifications and experience
Remuneration: $74,317.94 - $117,755.00 per annum, pro rata (+ super +17.5% leave loading where applicable)
Hours Per Week: up to 24

Our Ideal Candidate Will Demonstrate
- Tertiary qualifications in a health-related discipline (nursing, occupational therapy, physiotherapy, social work, or psychology) with current AHPRA registration or AASW membership
- Minimum of one year's experience directly delivering services in aged care settings or to aged persons in Australia or overseas
- Strong clinical assessment skills with the ability to assess diverse clients with complex care needs while meeting Commonwealth Key Performance Indicators
- Excellent communication and relationship-building skills to work effectively with clients, carers, multidisciplinary teams, and external healthcare providers
- Sound computer literacy with competency in Word and Excel for efficient data management and timely record-keeping
- Current Class C driver's licence with willingness and ability to travel regularly and over long distances for work purposes
