POSITION LOCATION: OWEN SOUND DEPARTMENT: ALLIED HEALTH UNION AFFILIATION: OPSEU – HPD STATUS: FULL-TIME, TEMPORARY (APPROX. END DATE SEPTEMBER 2026) WAGE RATE: $44.67 – $57.90 Reporting to the Manager of Allied Health, the Social Worker for the Rehabilitation unit is responsible to work collaboratively with other members of the interdisciplinary team to develop a plan of care that is based upon bio-psycho-social assessment with the goal of maximizing social and emotional functioning, preventing problems and situations that reduce coping capacities and independence as well as addressing barriers to optimal recovery or adjustment and discharge from hospital. Intermittent coverage for rural hospital(s) and rotational responsibilities within the Brightshores Social Work team.
Qualifications / Skills / Abilities
- Master of Social Work required
- Currently registered with the Ontario College of Social Workers and Social Service Workers or eligible for registration
- Working knowledge of Mental Health Act is an asset
- Psycho/social and mental status assessment skills
- Ability to provide a wide range of counseling, support, advocacy and education services to family members and others of significance in the patient’s life
- Ability to communicate positively and professionally with family members, community providers and physicians
- Experience in working with the Geriatric population
- Working knowledge regarding stroke is an asset
- Ability to provide coverage to other areas in the hospital intermittently
- Minimum five years of experience working in a multidisciplinary team environment
- Valid Class G driver’s license and willingness to travel
- Ability to work closely as a member of a team and share responsibilities
- Demonstrated ability to work autonomously with excellent organizational skills and scheduling abilities
- Able to work all shifts, 24/7, including weekends, and ability for multi-site coverage if required
- Recent satisfactory performance and attendance records
- Adherence to Brightshores ‘CARE’ Behaviours (Collaboration, Accountability, Respect, Excellence) and ‘LEAD’ Competencies (Lead by example, Empower, Achieve results, Develop others) and a commitment to a safe workplace free of violence and harassment
- Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills and attitudes
