About the role

Clinical Care Coordinator – Temporary 6 months

Lethbridge Family Services (LFS) is seeking a clinical care coordinator to join our team in Home Services! The clinical care coordinator provides case management for clients. They work as part of a multidisciplinary team to ensure client care is delivered according to organization procedures and industry best practices.

Clinical care coordinators:

  • co-ordinate service to each client based on authorization and service plans,

  • respond to client concerns, by phone and in person,

  • arrange and attend client meetings, maintain client files with current information, and documentation in accordance with organization policies and procedures,

  • report unusual occurrences and conduct root cause analysis,

  • conduct initial client visit and home assessment checklist and survey clients annually,

  • conduct in field visits with health care aides, twice per year, and

  • plan and schedule workers according to client needs, organization procedures, and funding requirements.

Qualifications:

  • a degree or diploma in, nursing or practical nursing. Current, active registration with a professional licensing body (CLPNA, CARNA) is considered an asset,

  • a minimum of 2 years of experience in care coordination or case management,

  • excellent organizational and administrative skills, and

  • excellent communication skills; verbal and written.

All new employees of LFS must provide a police information check and vulnerable sector check dated not less than 90 days prior to the date of hire.

Hours: 35 hours per week – including days, evenings, and weekends. Temporary position- 6 months.
