Bayshore Therapy & Rehab is looking for passionate and caring Social Workers to join our amazing team to assist with an innovative community crisis management program in Nova Socia. We are hiring full time salaried (1 year contract with potential to extend) and part time Social Workers in the Halifax Regional Municipality to work alongside other healthcare professionals 24-7. A Social Worker for Bayshore Therapy & Rehab will be able to adapt social work principles and practices to the community setting. This includes, but is not limited to, providing in person support for clients experiencing crisis, including mental health, suicidal ideation and homelessness to ensure safety and improve quality of life.

Duties and Responsibilities

Communicate with clients, service providers and other professionals to collaborate and coordinate services Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect them Utilize a client-centered approach to identify, prioritize and monitor potential psychosocial problems and goals and to de-escalate situations that could become a crisis. Deliver trauma informed client centered crisis intervention. Advise clients/caregivers on the availability of community resources and contact agencies on their behalf when appropriate e.g. Federal, Provincial, Municipal and Local agencies in relation to income security, pensions, financial assistance, etc.

Recommend and facilitate referrals to support services within the community

Developing a flexible, responsive system to meet changing and competing demands Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body Liaise with other Case Managers, funders or Government & hospital programs as required Liaise with Provincial regulating body as required Complete other tasks as requested

Requirements

Be an active member in good standing of the Nova Scotia College of Social Workers Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities Minimum of two (2) years of community-based therapy experience (preferred) Excellent command of the English language, both written and oral (French considered an asset) Be able to drive and hold a current valid driver’s license Provide a police clearance (Vulnerable Sector Check) that meets the Bayshore HealthCare policy Hold a current CPR certificate (CPR-C or BLS and is only acceptable a year after issue) Possess strong work ethic with commitment to regular attendance At Bayshore Healthcare, we are committed to fair, equitable, and transparent compensation practices.
