DETAILED JOB DESCRIPTION

Part of this job is efficiently coordinating all services a client with a developmental disability experiences at Prospect. You’re a born scheduler, planner, and coordinator. You would win, and probably organize, the Amazing Race, and you would do it so gracefully that the other teams wouldn’t say mean things about you to Phil off camera.
What will you do?
- Provide orientation to Prospect and program services, assess client goals and needs, schedule – coordinate and carry-out intake and planning meetings with clients and members of their support network, arrange for accommodations or assistive technologies where appropriate, determine acceptance of clients into service, develop employability assessments, service plans, employment profiles and client schedules.
- Act as the central point of contact for the client support network, from intake to completion of service.
- Monitor goal attainment, including soliciting feedback from other departments such as Employment Specialists, Employment Retention Specialists and Program Coordinators.
- Support other Prospect direct service positions in working directly with Prospect clients; providing support in order to help expand employability skills development
- Maintain knowledge of supports and resources within the community, including financial supports, funders, mental health service providers, supports for persons with developmental disabilities, crisis supports, behavioral supports, counseling supports and education and training supports.
What does it take to do this job?

Post-secondary education in a related field and 2 to 3 years’ experience with case management/service coordination in a similar industry or environment

Exceptional oral and written communication skills

Excellent organizational and analytical skills

Demonstrated proficiency with Microsoft Office

Prior experience with intake processes, employability assessments, accommodations, and service plans would be considered strong assets

- This is currently a hybrid role, offering the flexibility to work from home part of the time while also connecting with others through in-office collaboration.
- Those with a combination of post-secondary education along with professional experience may be considered
- A reliable vehicle and valid driver’s license is required for business travel.
